Responsibilities We are actively hiring for an Registered Nurse (RN) PreOp PACU. Promote the mission, vision, and values of SCA . Assess patient’s preoperative and postoperative status at intervals according to facility or health care organization policies and procedures to include the patient’s cardiovascular system, respiratory system and airway management needs, neurological system, hemodynamic stability, pain and comfort needs, and mental status. Interpret information accurately from various noninvasive and invasive monitors. Describe the underlying pathophysiology of commonly encountered disease processes. Describe the physiology associated with various anesthesia techniques and surgical or procedural interventions. Apply the relevant scoring system/discharge criteria for patient transfer ( e.g. , Aldrete Score, Modified Aldrete Score [ i.e. , PARSAP], Post Anesthesia Discharge Scoring System [PADSS], White) score accurately as defined by organizational policy. Develop an individualized perianesthesia plan of care. Implement the plan of care including the following interventions: Preparing for and managing patient recovery from anesthesia – general. Preparing for and managing patient recovery from anesthesia – monitored anesthesia care. Preparing for and managing patient recovery from anesthesia – regional. Preparing for and managing patient recovery from anesthesia – local. Preparing for and protecting patient from injury caused by positioning, thermal sources, and extraneous objects. C ommunicating the patient’s current status throughout the perianesthesia continuum of care. P roviding acute pain and comfort management as needed. A dministering medications safely and correctly by using the 8 -rights of safe medication administration. P erforming interventions to maintain the integrity of the patient’s wound and tissue perfusion at or above baseline levels . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems, and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect patient from infection. M anaging patient’s other co-morbid conditions as applicable. M anaging patient’s fluid as prescribed. A ssessing knowledge level of patient or designated support person and providing education regarding the expected psychosocial response, nutritional management, medication management, pain and comfort management, wound management, and expected responses to the operative or invasive procedure; I nvolving patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice; and P roviding consistent and comparable care regardless of the setting. Evaluate the patient’s progress toward attaining outcomes. Transfer care as needed. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions. Document nursing care completely, accurately, and legibly. Collaborate effectively with other disciplines as needed. Participate in quality review and performance improvement projects. PURPOSE AND SCOPE: This position is for graduates of an accredited school of Nursing working in a state that does not allow graduates to perform nursing duties until licensed. Functions as part of the hemodialysis health care team providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse while working towards obtaining their nursing license. PRINCIPAL DUTIES AND RESPONSIBILITIES: Patient Care: · Performs vascular access evaluation pre-treatment. · Performs and records Pre and Post dialysis evaluation, weight and vital signs with initial identification. · Performs vascular access cannulation and reports any difficulties in cannulation or unusual findings to nurse supervisor. · Initiates dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD). · Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed. · Monitors patients’ response to dialysis therapy. · Obtains vital signs prior to reinfusion; performs all relevant functions necessary for the discontinuation of treatment and documents appropriately. · Evaluates patients prior to termination of venous access - standing & sitting blood pressure. · Obtains hemostasis and applies appropriate dressings. · Evaluates the patient prior to discharge. · Documents information related to patient treatment include documentation in the patient health record. · Reports any significant information and/or change in patient condition directly to the nurse supervisor. · Performs and documents interventions for unusual patient status and document patients' response to intervention. · Obtains all prescribed laboratory testing and prepares specimens for collection. Maintenance / Technical: · Conducts all tasks necessary for preparation for dialysis treatment and documents where appropriate. · Maintains environmental integrity and aesthetics - ensures all areas are safe and clean. · Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy. · Performs all required pretreatment dialysis machine set up and testing. · Ensure that all blood spills are immediately cleaned with appropriate disinfectants according to facility policy. · Prepares, organizes, and efficiently utilizes supplies and equipment to prevent waste. Clerical & Administrative: · Enters all treatment data into the designated clinical application accurately and in a timely manner. · Reviews health care record for completeness and ensures omitted entries are completed or corrected where appropriate. · Prepares lab requisitions for prescribed or stat lab specimens according to laboratory destination. · Collects, labels, appropriately prepares and stores lab samples according to required laboratory specifications. · Ensures collection of lab specimens by appropriate lab courier. Responsibilities As a CRNA: Determines the optimal type and method of anesthesia care to administer for a broad range of surgical procedures at the MTF. Evaluates data, with respect to individualized anesthetic options and current practice guidelines, to determine and discuss the optimal anesthetic plan. Obtains patient consent for the type of anesthesia to be utilized, expected outcomes and possible anesthetic complications. Gathers equipment, prepares, inspects, and ensures proper sanitation and function of equipment to be used in performance of work. Serves as an anesthesia resource consultant to a wide variety of medical, surgical, and nursing specialties to delineate the latest advances in anesthesia practice. Evaluates data, with respect to anesthesia and evolvements within the field, to determine and discuss type of anesthesia to be used, and to explain expected outcomes and possible anesthetic complications. Determines necessity for laboratory studies, coordinates ordering of lab tests and evaluates results. Provides immediate, post-operative care to patients transported to the PACU, ICU or recovery area; ensures patient airway, hemodynamic stability, optimal pain control and appropriate level of consciousness. Provides immediate and appropriate interventions to include advanced life support (ALS) and cardiopulmonary resuscitation (CPR) according to guidelines and within the scope of practice.
