Nurse Practitioner/Physician Assistant (A) GYN/ONC - Royal Oak

The Advanced Practice Provider (APP) will primarily support the Women’s Health/Gynecologic Oncology outpatient practice at William Beaumont University Hospital in Royal Oak. This position is primarily based in Royal Oak; however, the APP may be asked to provide coverage at additional practice locations including Troy, Dearborn, and potentially Farmington Hills in the future. This is a full-time, Monday through Friday position with standard clinic hours of 8:00 a.m. to 5:00 p.m. The APP will be expected to maintain medical staff privileges at all practice locations and hospitals to support cross-coverage needs. Responsibilities will include outpatient clinical care, care coordination, and occasional inpatient rounding as needed. Preferred Qualifications Previous Advanced Practice Provider experience in Gynecologic Oncology, Oncology, or Gynecology preferred Experienced APP candidates strongly preferred New graduate APPs will be considered if they possess prior RN experience in Gynecologic Oncology, Oncology, or Gynecology Strong clinical, communication, and collaborative care skills Essential Functions Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. Determines eligibility of patients for entry into clinical trials based on findings. Requests written consultation from physicians and other healthcare professionals to ensure appropriate and quality patient care. Interprets and evaluates findings of studies/tests. Relays appropriate information regarding patient care to the collaborating physician. Administers therapeutic measures and obtains specimens as prescribed. Evaluates the quality of care provided and recommends changes for improvement. Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the client. Dictates follow-up letters to referring physicians with summaries of treatment, response, and plan. Maintains provision of preventive health services (e.g. screening, risks assessment, immunizations, PAP smears, self-breast exam). Dictates or writes patient history, admissions, care plans, progress, and discharge notes. Interacts with program assistants and data managers for optimal patient care. Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care. Conducts or participates in daily multidisciplinary rounds and ensures appropriate and quality care for assigned patient. Performs patient triage and determines the urgency of physician's evaluation. Provides symptom management for side effects related to oncologic treatment. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. The CLA performs a variety of tasks and services for the participants (residents), including: Personal care up to and including 2-person assist 1:1 interaction in DHC and CL as outlined in the plan of care Assistance with activities Assistance with meal preparation / cooking/ feeding/other nutritional needs Assistance with toileting / managing incontinence Simple treatments Grocery shopping Laundry services Coordination of care with participant and the DHC based on care plan Engage with resident elevating health concerns and customer services issues or needs appropriately Homemaking services / chores/ housekeeping services/sanitizing equipment All other duties as required by manager and lead CLA, as assigned Rounds q2 hour after 7pm-7am.
