Location New York, New York Shift: Day Flex (United States of America) Description: Staff Nurse - RN - Hospital at Home - Full time Days Hospital at Home is a unique and innovative program at NYP that provides inpatient hospital level care at the patient’s home. The program combines daily in-home visits, virtual visits, and 24/7 remote patient monitoring to give eligible patients an opportunity to stay in the comfort of their own home while receiving their hospital care. This program will be designed and delivered by NYP providers and clinical staff, allowing NYP's best-in-class care across all measures of quality, service, cost, and innovation to be continued in the home setting. Hospital at Home will provide strategic value to NYP by alleviating capacity constraints, improving patient experience, reducing readmission rates, and driving cost savings to the organization. Since 2020, CMS has authorized more than 320 hospitals across 133 health systems in 37 states to offer acute hospital care at home, and those numbers are climbing. The number of programs is expected to double by 2026. We are looking for motivated team members to help us become the largest, safest, and most innovative Hospital at Home program in the country. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Job Description The Department of Radiology at SUNY Downstate Health Sciences University is seeking an hourly/per diem Nursing Assistant 2. Distinguishing Characteristics Nursing Assistants provide care to individuals in hospitals and long-term care facilities operated by New York State. Care ranges from attending to patients’ personal hygiene needs, to performing a limited number of simple treatments and therapies under direction of licensed clinical staff. Incumbents provide direct hands-on care to individuals by assisting with bathing, dressing, perineal care, mobility, eating, and other daily living activities. Illustrative Duties Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Distinguishing Characteristics Nursing Assistants provide care to individuals in hospitals and long-term care facilities operated by New York State. Care ranges from attending to patients’ personal hygiene needs, to performing a limited number of simple treatments and therapies under direction of licensed clinical staff. Incumbents provide direct hands-on care to individuals by assisting with bathing, dressing, perineal care, mobility, eating, and other daily living activities. Illustrative Duties Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Operates a variety of specialized patient monitoring and treatment devices. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied.
