Description Title - Nurse Practitioner Manager, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Job Responsibilities: Provide clinical and administrative support for providers, patients, and office operations Greet and register patients as they arrive Obtain medical history, patient vital signs, and intake Prepare patients for X-rays Perform and document routine labs, such as throat and urine cultures (venipuncture experience preferred) Irrigate and assist with wounds Assist in performing quality control testing in the lab Be an integral part of the patient care team and assist with patient care within the scope of practice Keep exam rooms and front reception stocked with adequate supplies Maintain order and cleanliness of the waiting room area Practice standard safety practices and precautions Help patients in distress by responding to emergencies and notifying medical team (nurse, urgent care tech, and physician) Maintains operations by following policies and procedures; reporting needed changes to management team Maintains patient confidence and protects operations by keeping patient care information confidential Enhances health care practice reputation by accepting ownership for accomplishing new and challenging requests; exploring opportunities to add value to job accomplishments Assistant patients with complaints and taking an active role in service recovery when needed Perform other duties as needed including apply splints and assisting the nurse and/or provider in other medical procedures Provide exceptional customer services #LI-MO1 Qualifications High school diploma or GED Prior experience and/or education as a Medical Assistant Knowledge or EKG placement and processing in children Ability to obtain laboratory tests via nasal swab, and venipuncture Demonstrate a professional and friendly demeanor Strong communications and organizational skills Prior pediatric experience Capable of multi-tasking in a fast-paced environment Comfortable with computers, Microsoft Office Suite Willing to learn medical software Experience with medical records preferred Compensation: $18 - $26/hour The salary/rate range listed here has been provided to comply with local regulations and represents a potential base salary/rate for this role. Please note that actual salaries/rates may vary within this range above or below, depending on experience and location.
