Job Summary The Homecare Registered Nurse (RN) provides comprehensive nursing services to individuals, families, and groups in the home setting, delivering independent, patient-centered care with strong clinical autonomy in a community-based environment. This role coordinates the Plan of Care and applies all components of the nursing process under the Clinical Supervisor while ensuring safe, high-quality outcomes in the home setting. Registered Nurse (RN) Job Responsibilities: Provides skilled nursing care to patients in their homes (including admission/discharge) on a family centered basis while recording/maintaining complete patient record on Portable Electronic Device and/or paper record. Teaches, demonstrates, supervises and evaluates care given by peers, HHAs, family members and significant others. According to age and developmental level of the patient, systematically assesses the patient's physical, psychosocial, emotional, cultural, spiritual and discharge needs. Develops and implements a plan of care for each patient. Utilizes effective oral and written communication skills to include completeness and accuracy of documentation. Evaluates the plan of care and reformulates the goals as necessary. Demonstrates accurate technical skill and operational knowledge of patient care equipment and procedure, including venipuncture to administer IV medication, TPN, lipids, etc. Recognizes unsafe and life-threatening situations and intervenes appropriately. Job Responsibilities Providing post-acute LPN nursing care to residents with varying medical needs Tracking the vital signs, physical condition, and mental acuity of residents Completing intakes of residents, including the documentation of medical histories and health concerns Assisting with medications, including proper administration, verification, and monitoring Keeping track of the medical records and charts of residents Helping to maintain a safe, comfortable, and pleasant environment for residents Ensuring proper storage and inventory of medical supplies Setting up and assisting with immunizations, IVs, and phlebotomy procedures Being accountable to follow and adhere to care plans for residents and all applicable policies and regulations Supporting the training and orientation of new clinicians Qualifications and Skills Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN) Completion of approved Licensed Practical Nurse (LPN) educational program Graduation from high school (or GED/equivalent) At least 6 months experience working as a LPN (or similar role) Ability to lift at least 50 lbs Get the Work-Life Harmony You've Always Wanted There may be many Licensed Practical Nurse (LPN) jobs to choose from, but only IntelyCare offers you a full range of benefits and the freedom to grow your career and prioritize your personal needs.
