Job Description The APP will be responsible for the medical management of the hospitalized patient population within the scope of an Advanced Practice Provider. The term Advanced Practice Provider encompasses Nurse Practitioners, Physician Assistants, and Clinical Nurse Specialists. The employee will have successfully completed a Master’s degree (or higher level of educational program) and have met the requirements for the respective State Board(s) (Missouri and Kansas). The APP will hold and maintain a national certification and licensure as required by State Boards and Saint Luke’s Health System, as well as other certification as obligatory for this role. The PRN Hospitalist APP works 12 hours shifts overnight at the Saint Luke's Plaza and Saint Luke's locations. Weekend and holiday requirements with this role. This position is accountable for advanced clinical practice in the care of patients with general medical conditions, requiring acute inpatient treatment. Responsibilities include obtaining and analyzing patient histories, performing comprehensive assessments, developing and implementing treatment plans, coordinating care, and collaborating with physicians and interdisciplinary team members to achieve optimal patient outcomes. Job Description This Cardiology Advanced Practice Provider (APP) position is a full time role covering general cardiology and heart failure with the primary patient population being adult. Inpatient responsibilities will include general cardiology rounding at St. Luke’s South. Outpatient responsibilities will consist of heart failure clinics at both St. Luke’s South and St. Luke’s Hospital (Plaza). The schedule is Monday through Friday and consists of four 10-hour shifts (7:30 AM to 5:30 PM) each week, with one variable day off. There is a weekend and holiday commitment for this role. The employee will be responsible for the medical management of the Cardiology patient population within the scope of an Advanced Practice Provider. The term Advanced Practice Provider encompasses Nurse Practitioners, Physician Assistants, and Clinical Nurse Specialists. The employee will have successfully completed a Master’s degree (or higher level of educational program) and have met the requirements for the respective State Board(s) (Missouri and Kansas). PURPOSE AND SCOPE: This position is for graduates of an accredited school of Nursing working in a state that does not allow graduates to perform nursing duties until licensed. The specific job title used will be determined based on the state where the employee is employed. Functions as part of the hemodialysis health care team providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse while working towards obtaining their nursing license. Employees hired into this position will be required to obtain their nursing license within 90 days of their hire date. PRINCIPAL DUTIES AND RESPONSIBILITIES: Patient Care: Performs vascular access evaluation pre-treatment. Performs and records Pre and Post dialysis evaluation, weight and vital signs with initial identification. Performs vascular access cannulation and reports any difficulties in cannulation or unusual findings to nurse supervisor. Initiates dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD). Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed. Monitors patients’ response to dialysis therapy. Obtains vital signs prior to reinfusion; performs all relevant functions necessary for the discontinuation of treatment and documents appropriately. Evaluates patient prior to termination of venous access - standing & sitting blood pressure. Obtains hemostasis and applies appropriate dressings. Evaluates the patient prior to discharge. Documents information related to patient treatment including documentation in the patient health record. Reports any significant information and/or change in patient condition directly to the nurse supervisor. Performs and documents interventions for unusual patient status and document patients' response to intervention. Obtains all prescribed laboratory testing and prepares specimens for collection. Prepares, organizes, and efficiently utilizes supplies and equipment to prevent waste. Clerical & Administrative: Enters all treatment data into the designated clinical application accurately and in a timely manner.
