Principal Duties and Responsibilities: Assists in gathering data to assess the physical, psychological, social, spiritual needs of patients. Collaborates with the registered nurse and interdisciplinary/team members in planning for the needs of patients upon admission. Reviews the care plans and provides input into updating and revising. Supports in direct patient care to assigned patients utilizing clinical nursing skills and judgement. Provides education and reinforcement of education for patients and family, and documents outcome in the medical record. Initiates appropriate emergency measures when patient's condition dictates need. Prepares patients for tests and procedures. Administers and documents medications according to policy and procedure. Can appropriately delegate care to non-licensed personnel and report to the Registered nurse. Report to the Registered nurse with a thorough concise report on patient condition changes or evaluation of interventions. This Provider Informaticist I position will be based in the Baltimore area and focused on supporting ambulatory Oncology. Ideal candidate will be an RN with Oncology experience. Primary Duties and Responsibilities Assists in establishing and maintaining a therapeutic and safe environment that facilitates and supports the framework for professional clinical practice based upon evidence-based practice in order to achieve quality patient outcomes. Utilizes approaches and strategies in collaboration with the healthcare team that support and optimize patient care outcomes that are based on the interdisciplinary plan of care. About this Job: General Summary of Position This leadership role is generally home-based but requires travel to all four of MedStar's Baltimore-based hospitals for leadership meetings and community-based events. Candidates should have a strong knowledge of case management and community resources. Serves as the leader of the Case Management/Transitional Nurse Team.
