Monday to Friday, 730am-400pm Summary: Position Summary: To provide nursing services to patients at a provider’s office. Patient care includes care to patients in following age group: pediatric infant to 12 years , adolescent 13 to 17 years , adult 18 to 64 years and geriatric over 64 years . Qualified individuals must have the ability with or without reasonable accommodation to perform the following duties: Provides clinical advice by phone electronic correspondence based on clinical judgments and or triage protocols in accordance with regulatory requirements and standards of practice. Consults with provider as necessary. Provides patient specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications and other areas pertinent to patient outcomes. Prepares patient for visit by using appropriate rooming standards tool. Administers injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements and standards of practice. Prepares and assists providers with various procedures, minor surgeries and other exams as designated by the provider. Performs Point of Care Testing, including but not limited to hearing, flu, urine and strep tests. Collects blood or other laboratory specimens and prepares them for further testing. Authorizes prescription refills as instructed by the provider. Assists with test tracking and monitoring overdue results. Removes sutures and changes dressings on wounds as necessary. Assures exam rooms are adequately supplied and cleaned after each patient visit. Maintains and cleans medical instruments and equipment after each procedure. Assists provider and patient in reviewing and revising care plans and assesses patient progress. Follows up with patient between visits as necessary. Assists with proactive outreach for patients requiring preventive and chronic disease services. Performs care coordination activities among specialists and transitions of care to obtain all necessary patient information. Assists in coordination of referrals, education and community resources. Actively participates in performance improvement initiatives to improve overall patient experience and quality of care. Participates in daily huddles with the provider. Serves as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors and colleagues. Communicates with all patients, visitors and colleagues in a professional manner while keeping customer service a top priority. Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures. The following duties are considered secondary to the primary duties listed above: Performs various front office responsibilities as necessary. May require some regional travel - valid driver's license is required if traveling. Participates in the on-call responsibilities for this position as required. Other duties as assigned. Manager will support mobile health units across the Delaware Valley (DE/PA/NJ/MD) This Position Is Responsible For Leading the end-to-end planning, deployment, and continuous operation of pediatric mobile healthcare services that expand access, improve outcomes, and strengthen community trust across DE, PA, MD, and NJ. Facilitates, coordinates and directs patient care services and provides leadership for the Nemours mobile health unit. Partners with ambulatory nursing and specialty division leaders/service lines to ensure access on mobile unit and into the community is available on a regular cadence. Job Description Nemours is seeking a Clinical Nurse Supervisor to join our Nemours Children's Health team in Wilmington, DE for a full-time, rotating days position (7A-7P and 11A-11P). This is a registered nurse with clinical and shift management responsibilities for hospital operations. Position Responsibilities Shares clinical expertise with staff and provides direction for staff in exercising clinical skills. Demonstrates management skills effectively and universally when dealing with staff on a shift. Guides clinical practice by adhering to the Nurse Practice Act. Initiates cost-effective and appropriate quality care by: Reviewing and adjusting unit assignment of staff in collaboration with Nurse Managers/Charge Nurses to insure adequate coverage of all patient care units in a cost effective manner. Identifies problems on a shift and initiates actions to correct the problem. Communicates problem areas to Chief Nurse Executive/designee. Enters all staffing changes in the automated staffing and scheduling system. Interprets and role models Hospital and Department philosophies, policies, and objectives. Provides Clinical management coverage for all patient care units. Informs and reports any unusual occurrences to the Chief Nurse Executive /designee and Administrator on-Call. Functions in the role of the Incident Commander during a Code Delta until relieved by the Administrator-on-Call. Manages and resolves all patient care issues in the absence of the Chief Nurse Executive/designee. Identifies problems, gathers sufficient data, analyzes data, and determines strategies to utilize for problem resolution. Contributes appropriately to quality improvement activities. Evaluates outcome of problem resolution to determine effectiveness. Supports development of staff by encouraging participation in educational programs. Identifies individual and unit educational needs for a specific shift on weekends and communicates to Nurse Managers/designee on Nursing Staff Development. Communicates staff deficiencies to the unit Nurse Manager. Provides periodic evaluation of staff to the unit Nurse Manager. Provides constructive guidance to staff for development of clinical expertise. Counsels employees in the absence of the Nurse Manager. Demonstrates ability to work collaboratively with ancillary departmental staff. Establishes a positive work environment to achieve optimal patient care. Demonstrates effective and professional written and oral communication skills. Conveys unit information regarding patients, unit activity, staffing, and personnel problems to the Nurse Manager/designee daily. Provides recommendation to improve nursing work flows and efficiency. Performs all other related tasks as indicated by the Chief Nurse Executive/designee. Responsible for participation in departmental and hospital programs for Quality Assessment and Improvement, identifying opportunities to improve services, making recommendations and implementing actions as appropriate and consistent with the goals of the Nemours Foundation. Position Requirements BSN required American Heart Association BLS required Active Delaware (or compact state) Registered Nurse Licensure Required 3-5 years experience as a staff nurse.
