This is an independent contractor (1099) position offering approximately 6 flexible hours per week, including one scheduled on-site visit each week. Most documentation may be completed either on-site or remotely at your convenience. What you will be doing: Complete routine health assessments, including resident vital signs. Review medications, medication administration practices, and Medication Administration Records (MARs). Consult with Direct Support Professionals (DSPs) and management regarding resident health concerns and medical recommendations. Monitor residents' overall health status and identify follow-up needs. Additional Responsibilities Include: Complete required monthly and quarterly nursing reports. Provide medical consultation and guidance to management and staff. Provide telephone consultation for urgent medical and medication-related questions as needed. Additional time spent responding to calls beyond the contracted six hours is compensated at the hourly rate. Train newly hired staff on medication administration and evaluate medication competency. Help ensure compliance with applicable ICF regulations and standards. Current California Registered Nurse (RN) license in good standing. Strong clinical assessment and medication management skills. Excellent communication and collaboration abilities. Ability to work independently and manage a flexible schedule. Experience with Intermediate Care Facilities (ICFs), residential services, or individuals with intellectual and developmental disabilities is preferred. Knowledge of California ICF regulations is a plus. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Care provided must be documented in the electronic medical record in accordance with the UCSF Rules and Regulations Provider services must be billed to reflect the performing provider and comply with the APP Billing Policy 3.08.01 . May first-assist in the operating room if privileged to do so through OMAG after completing the required certification and proctoring. 1.3. Identifies high-risk groups and complex cases for review with attending physician. Initiates consultations as appropriate. 1.4. Evaluates complex cases, establishes individualized plans of care for rehabilitation, and facilitates occupational and physical therapy and physical medicine and rehabilitation consultations. 1.5. Interprets and acts upon the ordered tests, procedures, and radiographic studies in a timely fashion. 1.6. Completes clinically relevant paperwork/EHR, including but not limited to ordering medications, completing episodic care notes and telephone encounters. Outpatient – Care Coordination (may include some or all of the responsibilities listed) ## 1.1. Coordinates care throughout the continuum including hospital admission, care coordination with staff and consulting services, and hospital discharge planning. DOES not include management of inpatients. 1.2. Coordinates episodic care and determines timing of routine clinic follow up. 1.3. Participates in huddles with clinic/hospital staff and anticipates patients who require close management or intervention. 1.4. Attends department quality improvement meetings and participates in patient evaluation and planning. 1.5. Consults with nursing staff on plan and implementation of individualized nursing care plans. 1.6. Independently provides case summaries for disability, insurance agencies, workers compensation, work releases, and medical supply agencies. Total 100 Qualifications Required Qualifications: Graduation from a California Board of Registered Nursing recognized Nurse Practitioner Program with a master’s or doctoral degree aligned with an Acute Care Pediatric Nurse Specialization Nurse Practitioner 1: Minimum of two (2) years of recent experience working as an RN in a Pediatric ICU setting Nurse Practitioner 2: Minimum of two (2) years Nurse Practitioner experience in a pediatric inpatient or emergency department setting AND minimum of two (2) years inpatient RN experience in pediatric critical or transitional/intermediate care OR Minimum of 1 year of NP Experience in Pediatric Critical Care OR Completion of a Pediatric Critical Care APP Fellowship Program Preferred Qualifications Minimum of three (3) years of experience working as an AC-PNP in a level III Pediatric Intensive Care Unit Licenses/Certifications California Registered Nurse (RN) License without restriction California Nurse Practitioner (NP) License without restriction California Nurse Practitioner Furnishing Number NP national board certification from an accepted certifying agency aligned with the patient population and clinical role - Pediatric Inpatient (CPNP-AC). California Driver License (if required for position) Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address American Heart Association (AHA) Basic Life Support (BLS) certification Additional required AHA certifications: PALS Additional required certifications (such as ENLS): None UCSF Medical Center requires all Advanced Practice Providers (APPs) to be credentialed through its Office of Medical Affairs and Governance (OMAG) to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings. Credentialing takes between 60-90 days and must be completed prior to the first working day. You will be assigned an FPPE template of procedures for CRNAs, but you may add additional privileges on request with an explanation of the purpose.
