Job Summary The Certified Nursing Assistant (C.N.A) functions within the Scope of Practice outlined by the Maryland Board of Nursing. The C.N.A assists the Registered Nurse and interdisciplinary team in the safe delivery of quality patient care, while supporting unit operations. Unit Description: 3A is a 26-bed unit dedicated to the management of adult patients with a range of orthopedic and neurological conditions. The unit primarily serves patients recovering from joint replacement surgeries and hip fractures, as well as those requiring acute care for neurological conditions including stroke and seizure disorders. Our unit is staffed by a highly skilled nursing team with expertise in both orthopedic and neurologic care. Several nurses hold specialty certifications in stroke care and medical-surgical nursing, reflecting advanced clinical knowledge and a commitment to evidence-based practice. All nursing staff maintain Advanced Cardiac Life Support (ACLS) certification, ensuring rapid recognition and response to clinical deterioration. Care delivery on the unit is centered on a multidisciplinary approach, promoting optimal recovery, patient safety, and functional outcomes. Nurses are trained to perform focused neurological assessments, post-operative monitoring, early mobility initiatives, and patient and family education. The team collaborates closely with physicians, rehabilitation services, and ancillary staff to support individualized care plans and coordinated discharge planning. The unit emphasizes high-quality, patient-centered care with a focus on safety, early intervention, and rehabilitation to support patients in achieving their highest level of independence following hospitalization. Essential Functions: • In a team relationship with a registered nurse, and under the instruction, direction, and supervision of a nurse, the CNA shall: o Collect and record routine health data that is defined but not limited to cultural, religious and socioeconomic background and activities of daily living. o Orally report the health status data as required by the nurse. o Provide feedback to the nurse regarding the nurse-directed plan of care, which is being implemented. o Provide feedback to the nurse about the patient’s expressed wishes concerning the plan of care. • Provide assistance with activities of daily living as delegated by the nurse, including but not limited to: o Morning, evening, and general care to include bathing, mouth care, hair care, shaving, and skin care according to individual patient needs. o Repositioning patients to prevent skin breakdown. o Ensuring patients are out of bed and ambulated according to physician orders and RN direction. o Preparing for and assisting patients with meals, nourishments and beverages. • Perform interventions as delegated by a registered nurse, including but not limited to: o Hourly rounding to include the 4 P’s: Pain, Potty, Personal Needs; Possessions. o Blood glucose testing, vital signs, pulse oximetry, height, weight, SCD’s, indwelling urinary catheter removal, indwelling peripheral intravenous catheter removal, simple wound care to uncomplicated healing surgical wounds, and/or other equipment required by each department. • Record routine data and care provided on appropriate documents. • Identify the need for assistance and request assistance when the CAN is unsure about the condition; does not know how to do an activity; or is working in a new practice setting. • Report the patient’s response and the patient’s progress to the nurse at designated times and/or when there is a change in the patient’s condition. • Participation in: o Quality of care activities o Performance self-appraisal o Continued learning o Collaboration o Completion of appropriate delegated assignments. • Adhere to local, state and federal guidelines and regulations regarding client confidentiality and privacy rights. The nurse will utilize this time to learn the institution and nursing practice. . It is expected that a nurse in this role successfully meets all requirements of the Nurse Residency Program and Departmental Orientation. Utilizes the institution’s care delivery and professional practice models as the frame of reference for practicing as a professional nurse. Participates in activities to develop own practice, and to support group practice goals at the unit level. Is accountable to patient, family and team members for care provided and to the nurse manager for totality of work performance. Is accountable to hold to the Service Standards and to the Nursing Mission, Vision and Values. Responsibilities II. Clinical Practice/Care Delivery Focus of clinical practice/care delivery and on own learning. Uses/applies evidence-based practice, accesses appropriate resources. Follows unit based protocols, documentation, pathways, etc Involves patient and family in care; follows model of care delivery guidelines. Conducts patient/family needs assessment; initiates and documents patient/family education, seeks guidance in advocating the patient/family needs when appropriate. Delivers care consistent with unit operations to ensure safe, timely, effective, efficient, equitable, patient centered care. Professional Development Responsible for accessing education applicable to own professional development; focus on completion of competencies appropriate for patient care setting. Successful completion of orientation. Accountable for achieving/maintaining requirements for unit practice. Assists with precepting students, unlicensed assistive personnel or peer for isolated days (not accountable for entire orientation); may assist with share days or shadow days. Completes CE’s on an annual basis based on the facility minimum requirements. Service/Quality Becomes familiar with National Hospital Quality Measures, National Patient Safety Goals, Quality Indicators, Nursing Sensitive Indicators, and Facility Annual Operating Plan. Aware of patient and team member satisfaction scores and contributes to unit initiatives for improvement. Participates in some or all elements of research/EBP/QI. Provides high quality, safe, patient centered care with focus on exceeding service expectations. Maintains regulatory requirements for overall readiness; participates in tracer activities when requested. Patient Safety Takes action to correct observed risks to patient safety. Reports adverse events and near misses to appropriate management authority. Implements policies, procedures, and guidelines consistently in the performance of assigned duties. Develops effective working relationships and maintains good communication with other team members. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge. Participates in hospital, departmental and/or unit patient safety initiatives. Operations Supports Charge Nurse role and demonstrates basic understanding of the role. Actively contributes to teamwork (cooperates with peers, flexible with assignments, takes admissions as required, etc). Attends briefings/staff meetings regularly. Aware of and supports unit, nursing division, and organizational governance structure. Participates in recruitment and retention activities at the unit level. Provides feedback in peer review as requested. Work Experience Work Experience III. Qualifications Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required. 12 months or more of unsupervised RN experience Current AHA BLS certification required. Basic computer skills are required. Effective verbal and written communication skills. About this Job: General Summary of Position Functions as a member of the patient-centered care team to provide direct patient care activities under the supervision of a Registered Nurse for assigned patients and their families/significant others. Assists medical and nursing staff with procedures and/or tests and patients with Activities of Daily Living (ADLs). Performs specific care activities such as specimen collection electrocardiography (EKG) and documentation in the health record.Incumbents in this position may perform all or a combination of the duties described depending upon the assigned work area and the specific needs of the patients and the department. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality and safety standards and SPIRIT Values. Complies with governmental regulations and accreditation standards. Assists patients with activities of daily living (ADLs) and personal hygiene including bathing mouth care dressing skin care positioning feeding ambulation toileting and range of motion activities. Assists with patient mobilization and rehabilitation use of crutches and walkers and transfer/transport of patients in wheelchairs and stretchers. Collects specimens (e.g. blood urine sputum stool); and obtains EKGs. Assists in the measurement and documentation of patient's intake and output (I&O). Accurately takes and records patient's vital signs height and weight pulse oximetry point of care and waived testing (i.e. finger sticks for glucose monitoring/Accucheck urine dipstick hemoccult) and other pertinent data or information in the patient's health record. Performs routine sterile non-sterile (e.g. application of thermal soaks) and other treatments/activities identified by Unit/Hospital policy inclusive of but not limited to: tube feedings; ostomy care; Foley Catheter care; perineal care; simple dressing changes and wound care; turning and positioning; postmortem care and transportation of body to the morgue; and assistance with the application maintenance and removal of traction casts splints and exercise equipment. Maintains the comfort privacy and confidentiality and dignity of each patient in the delivery of services. Performs patient observation duty including 1:1 observation and safe application of restraints. Prepares patients to be seen by providers. Assists providers and registered nurses in patient examinations and procedures. Rounds on all assigned patients based on established standards (e.g. hourly purposeful rounding). Reports actual and potential problems concerning patients/families including changes in the patient's condition to the registered nurse and other care team members as necessary. Ensures complete hand off to oncoming shift and obtains full hand off from outgoing shift. Initiates and assists in CPR when indicated. Participates in patient/family teaching as directed and responds to questions from patients families and visitors in a prompt courteous and respectful manner referring them to appropriate personnel if necessary. Maintains the cleanliness and safety of patient rooms/environment by distributing and changing linen/towels removing used meal trays checking bed siderails and safety restraints etc. Assists with environmental services support such as linen management trash removal and cleaning patient rooms as needed. Transports and/or escorts patients during discharges transfers and/or medical appointments. Completes errands such as delivering specimens to the laboratory equipment and supplies etc. as required. Receives and stocks equipment and supplies and maintains orderly supply and utility rooms. Attends staff meetings ongoing educational offerings and participate in multidisciplinary quality and service improvement teams. Minimal Qualifications Education High School Diploma or GED or equivalent required Completion of an approved nursing assistant certification program required Experience Experience as a Certified Nursing Assistant preferred General health care experience in an acute care long-term care or other settings preferred Student nurse in junior or senior year preferred Licenses and Certifications CNA - Certified Nursing Assistant from the relevant jurisdiction required CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required Additional unit/specialty certifications may vary by department or business unit.
