This is a permanent, full-time job. Monday - Friday, 38.5 hrs/week. Sign-on Bonus of $15,000 paid after 15 months of employment. In this role you would be responsible for traveling to a patient’s home to administer healthcare services and helping patients maintain their independence. Caseload - 20-25 patients per week.
