SUMMARY Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications for students in the school programs, under the direction of a Registered Nurse and in accordance with company policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Licensed Practical Nurse - Children's Services will include, but are not limited to the following: Serves and participates as a member of interdisciplinary team. Is responsible for assisting Registered Nurse in coordinating and following through on provisions of health care services for students in program. Keeps progress notes under supervision of registered nurse for each student and summarizes for quarterly, semi-annual and annual reviews. Provides regular and PRN health care to students receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to staff, students receiving services and families as required. Handles control of communicable diseases and infections through identification and assessment. Reports to consulting physician and Infection Control Nurse. Implements appropriate protective and preventive measures. Provides staff training in regard to infection control regularly and as needed. Provides first aid to students in need of immediate care, with referral to registered nurse, private physician or outside emergency services as necessary. Monitors required staff health records and provides tests in accordance with regulatory agency requirements. Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data of students via progress notes. Administers medications as prescribed. Supervises secured storage of medications. Refers all consults and diagnostic reports to registered nurse for review. Transports students to appointments as necessary. Maintains Nursing Office. Orders supplies and equipment as required. Visits students at home as requested by Director or designee. Keeps abreast of changes in Nursing Services field. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Communicates effectively with the patients family or caretakers; Participates in planning for discharge and coordinates referrals, as appropriate; Provides discharge instructions to patient and significant other. * Evaluates care provided for patient outcome Demonstrates ability to measure effectiveness of care provided and documents same; Performs variance analysis related to outcome data for performance improvement; Designs, implements, and evaluates systems to improve care in Unit; Keeps accurate documentation of patients treatment, activity and condition; Uses appropriate methods of documentation according to departmental policy. * Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team Collaborates with the appropriate physician on patients plan of care; Accurately interprets and implements treatment regimen as prescribed by the Physician; Assists the Physician during treatments and/or diagnostic procedures; Keeps the Patient Care Manager/designee and/or physician, abreast of changes in patients condition and/or treatments, as appropriate; Uses clinical judgment in delegating assignments to the Patient Care Associate, Support Care Associate and Clerical Service Associate in providing patient care, and ensures that assignments are completed in a timely fashion. * Performs grade I-IV Decubitus Care Performs preventive skin care measures Applies simple dressings, maintaining principles of aseptic technique; Applies warm & cold compresses, consistent with facility procedure; Performs irrigations, consistent with Facility procedure; Performs other procedures related to skin care, as necessary. * Administers medications correctly and safely Correctly identifies medication: action; dosage; side effects and implications for the patient; Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V. drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. * Participates in patient and family education Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-hospital care; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN Degree within five (5) years of employment date. Current license to practice as a Registered Professional Nurse in New York State. BCLS, ACLS, preferred. Minimum of 1 year of ED experience preferred.
