CareMed COMPANY DESCRIPTION CareMed Health Solutions is a facility-based team of medical providers who provide medical services to patients living in long-term care facilities. We strive to give quality care to residents while working together with the facility to carry out the best care possible. While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. POSITION OVERVIEW The Evening Supervisor (LPN, RN) is primarily responsible for assisting the Director of Nursing (DON) and Assistant Director of Nursing (ADON) in overseeing the evening functions of the Nursing Department(s) to ensure that the highest degree of quality care is always maintained. Assist in the scanning and attaching of resident documentation in MatrixCare to ensure Medical Records are complete and accurate. Maintain first impressions to welcome potential customers to experience our services and participate in survey and mock-survey campus preparedness. Ensure campus is tour ready including back up team resources, tour path, model room, first impressions, and tour stations. Execute tours as necessary. Provide patient care as necessary. Ensures that residents' who are unable to call for help are checked frequently. Gives direct resident care such as bathing, dressing, and grooming of residents. Explains procedures to residents. Assists residents as necessary in their activities of daily living. Assists residents with bowel and bladder functions. Assists residents in preparation for and transporting to appointments, activities and social programs. Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs. Provides incontinent care as needed. Dresses resident appropriate to the current weather. Notifies Supervisor if resident needs clothing. Changes bed linens. Handles linens according to infection control guidelines. Feeds residents requiring help and records meal intake. Delivers and documents all snacks and supplements. Offers fluids to residents frequently to ensure that residents are hydrated where designated by care plan. Reviews and implements daily nursing needs as outlined by resident Care Plan or nurse managers. Transports residents or assists residents with mobility needs. Turns and repositions bedfast residents, alone or with assistance. Takes and records temperatures, blood pressure, pulse rates, respiration rates, height, weight, and food and fluid intake and output, as directed. Monitors resident's activities and reports any irregularities of resident performance to nurse managers. Participates in and receives nursing report upon reporting and departing to / from work. Reports all changes in residents' conditions to supervisor. Cleans instruments and equipment and returns to proper area after use. Maintains cleanliness of residents units, central bathing area, treatment and supply rooms, workrooms, and linen closets. Provides post-mortem care as instructed. Accurately records all care as directed. Participates and attends periodic in-services and staff meetings. Understands and adheres to the guidelines of "Residents Rights". Perform additional duties as assigned. EDUCATION / REQUIREMENTS: Ability to communicate with residents and nursing staff. Ability to speak, read, and write English. Prior experience as LTC nursing assistant preferred; or equivalent combination of education and experience. Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program. Good standing in the state registry is required.
