Under the direction of the RN, the Psychiatric Nursing Assistant (PNA) performs the activities related to the implementation of nursing care plans and the nursing standards, policies and procedures. The PNA is responsible for contributing to the development, evaluation and reassessment of care plans through observing and reporting changes in patient behavior and physical status. The PNA is also responsible for assisting in the maintenance of the safe and therapeutic environment of the unit. The individual is competent to care for patients in the following age groups: adult (ages 19 - 64) and geriatric (ages 65 +). Demonstrates an understanding of, and embraces, the mission statement of the Christian Health Care Center. Competencies: Supervises or assists in after meal care (removing trays, encouraging patients in brushing teeth, washing up, etc.). Serves food trays. Assists with feeding as indicated. Assists patients in preparing for activity and social programs, as per unit program schedules. Answers call bells promptly. Changes bed linens. Assists or supervises patients to walk with or without self-help devices as instructed. Reports all changes in the patient's condition to the Charge/Staff Nurse as soon as practical. Keeps floors dry. Reports spills immediately. Maintains intake and output records as instructed. Greets and escorts patient to his/her room. Informs the Charge/Staff Nurse of equipment and supply needs. Records all supplies and equipment utilized for appropriate procedures to Charge/Staff Nurse. Assists in transporting patients to/from appointments, activities and social programs, etc. as necessary/assigned. Maintains the confidentiality of all patient care information. Schedule: Part-time, 7am-3pm, Every other weekend. Education: High School graduate or equivalent preferred. Responsibilities POSITION SUMMARY Plan, implement and evaluate nursing care provided, including restorative care for residents in a manner that is reflective the ESCC Core Values. As the direct care provider of health services, he/she shares appropriate information with all other health services staff in an effort to assist each resident in achieving his/her maximum level of development. Directs and supervises CNAs, unit a clerk, LPN and other RN’s when in charge. Establishes effective working relations with staff residents and family members. Ensures that the Center and the Department of Nursing policies and procedures are follow in the planning implementation and evaluation of care. Review medication administration records for completeness at end of shift. Give and receives the nursing report and follow through on any special instructions. When in charge makes periodic checks to ensure that prescribed treatments are being properly administered by LPN’s and CNAs and to evaluate the residents physical and emotional status. Participates in the orientation of new staff residents and families to the unit. Carries out restorative and rehabilitative programs including teaching resident to wash/feed/dress self as appropriate. Ensures that the facility policy on the use of restraints is followed and that documentation is timely and accurate. Ensure that the unit is kept clean free from clutter and that the supply room door is kept locked; request the supervisor’s intervention as necessary to achieve this. Follows infection control standards and ensures that assigned personnel do likewise. Keep abreast of new nursing policies and procedures. Keep nurses notes, plan of care and all other documentation pertaining to your assigned resident timely accurate and complete. Carries out end of shift responsibility e.g. supplies are available for next shift’. Unit is clean and tidy; all documentation is complete and accurate. Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator. Qualifications Education Requirements: Graduate of an accredited RN program. New York State RN License Required New York State RN Registration Certificate Required Bachelor’s or Master’s degree in nursing preferred Current BCLS certification Experience One (1) year clinical experience in a pediatric and/or adolescent chronic /rehabilitative care facility preferred. Employee Requirements: Must be able to read, write, speak, and understand the English language fluently. Must meet or exceed all health standards and requirements for the position as established by NYDOH and all other regulatory agencies. Must meet the general health requirements set forth by the policies of this Center which includes a medical and physical examination. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Provide or assist residents with activities of daily living. Performs therapeutic procedures for children such as bathing, grooming, playing, feeding, positioning as per policy and procedure. Observe, report and record abnormal vital signs, temperatures, seizures activity, vomiting, diarrhea, behavior changes or any other abnormalities to the RN or LPN in charge. Inspect resident’s skin and promptly report any changes to the NCC. Carries out mouth care, hair care and as necessary trim finger and toe nails. Make two hourly checks on incontinent residents and wash and change as necessary. Encourages self-help measures as appropriate so that each child may reach his/her appropriate level of independence. Check and record urine and bowel movements. Ensure that assigned residents are appropriately prepared for school, clinic, recreation and hospital visits as scheduled and accompany/transport them as assigned. Ensure that bedfast residents are repositioned as directed by NCC but not less than two repositions per resident hourly. Performs restorative and rehabilitative activities as instructed and as directed by Plan of Care. Assists in applying safety restraints to residents when directed; Monitors residents in restraints, release restraints and documents according to policy and procedure Ensures that each resident who can use a call bell has one within reach and remind him/her to use it. Assists co-workers with lifting, positioning and other procedures as necessary. Be aware of facility policy on child abuse and/or neglect and promptly report any instances of abusive or inappropriate behaviors towards any resident to the NCC. Knows and follows The Center policy and procedure with regard to infection control including hand washing and types of isolation. Keep resident care area clean and free of clutter. Report any unsafe conditions immediately to NCC. Properly stores and safeguard residents’ clothing and reports too many or too few clothing and or toys to NCC. Feeds residents or teaches residents to feed themselves as appropriate. Ensure that hands are washed (child and caregivers) and that meal times are pleasant, dignified experiences Qualifications High School Diploma or equivalent required Successful completion of NYS approved Nurse Assistant Certification Program Active CNA Certification on NYS Prometric Nurse Aide Registry required Experience Experience working with children. Must be able to push, pull, move, and/or lift a minimum of thirty-five (35) pounds. Perform tasks that may involve exposure to the resident’s blood/body fluids. Must be able to assist in the evacuation of residents, if necessary. Working Conditions: Will work in a well-lighted and ventilated area. Will be subject to frequent interruptions. May be asked to work beyond normal working hours, and in other positions temporarily, when necessary.
