Manage chronic conditions, monitor patients' progress, and adjust treatment plans as necessary to optimize health outcomes. Health Promotion and Disease Prevention: NPs/PAs will educate patients and families about health promotion strategies, disease prevention measures, and lifestyle modifications to improve overall health and well-being.
