About this Job: Inpatient Psychiatry RN Case Manager (Discharge Planner) The Inpatient Psychiatry Unit (5 West) at MedStar Georgetown University Hospital offers in-depth evaluation and acute care treatment of the broad range of psychiatric disorders. 5 West coordinates all patient care through an interdisciplinary team of healthcare professionals, including psychiatrists, nurses, mental health counselors, occupational and art therapists and case managers. The unit houses a 13-bed, acute care, voluntary adult program and is located on the fifth floor of the Main Building. Key Responsibilities As a Clinical Nurse, you will provide skilled, compassionate care to patients and families across a spectrum of needs—from routine to highly complex. You will assess evolving patient conditions, identify actual and potential health concerns, and develop appropriate care plans. The RN Case Manager (Discharge Planner) serves as a member of the Case Management Team and applies RN clinical expertise and medical appropriateness to care coordination and discharge planning. Facilitates the delivery of quality cost effective patient-centered care from pre-admission through post-discharge timeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay promotes effective case management and utilization of resources and works to achieve optimal clinical and resource outcomes for the acute and post-hospital phases of care.
