Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Professional Development 1. Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. 2. Advances clinical competence in nursing practice to progress from novice to expert. 3. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. The Division of Family and Child Health (DFCH) of the New York City Department of Health and Mental Hygiene is charged with the creation and oversight of programs, policies, services, and environments that support physical and socio-emotional health, and promote primary and reproductive health services, health equity, social justice, safety and well-being for New York City families and children. The salary range for the role is $96.4228 - $96.4228 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Examples Of Typical Tasks Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. Participates in defining, implementing, maintaining and interpreting standards of nursing practice. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. Participates in determining conditions, resources and policies essential to the delivery of nursing care services. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Performs other professional nursing activities as delegated by nursing service administration. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
