A Brief Overview Coordinates and case manages total care of Home Care/Hospice patients, instructs and supervises Home Health Aides. Attends nursing staff meetings and in-services as scheduled. PURPOSE AND SCOPE: Ensure the provision of quality patient care to all hospitalized patients in the most efficient manner in accordance with company policy. Provides direct supervision and coordination of inpatient staff. Responsible for mid volume monthly treatments. Ensure all acute treatments are performed in contracted hospitals according to the established policies and procedures and in compliance with all applicable regulatory requirements. Ensure efficient utilization of supplies and equipment. Assist with various projects as assigned. Other duties as assigned. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Appropriately enters orders and schedules appointments in the Practice Management System (PMS) and EHR. Provides referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR. Appropriately adheres to universal safety precautions when administering medications and disposal of medical waste. Maintains proficiency with department specific equipment, troubleshoots and reports malfunctions promptly Facilitates transition of care to UCC/Hospital Demonstrates flexibility with various work schedules. All other duties as assigned Recognizes and performs duties which need to be performed although not directly assigned. Provide scribing services. Education, Certification, Computer & Training Requirements: High School Diploma or equivalent, required. Graduated from an accredited medical assistant or phlebotomy program, required. 0-1 year related work experience, required. 2-4 years related work experience, preferred.
