Job Description The Institute of Continues Learning in the Department of Nursing at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Wound and Ostomy Care Nurse. The successful candidate will: Develop and manage an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance Demonstrate knowledge of up-to-date topical wound care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapy Provide hands-on wound care to patients, including debridement of wounds by enzymatic and autolytic methods when appropriate Order, review, interpret; and integrate diagnostic studies and laboratory findings for initiating or modifying the treatment plan Collaborate with the patient primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan Update the primary care provider on the status and management plan of the wound as appropriate Prescribes medications and/or durable medical equipment to assist in healing wounds Assist with the identification of wound care quality measures and provide data and documentation for tracking quality measures Work with nursing leadership and the Director of Nursing Education to determine opportunities for education of nursing staff with appropriate identification of wound types, prevention of wounds, and nursing care of wounds Develop wound care education plans in collaboration with nursing leadership Instruct patient/family regarding wound treatments and management plan for optimal wound outcomes Triage telephone calls and provides consultation and serves as a resource to the frontline nursing staff Participate in professional development activities and maintains professional affiliation Qualifications Required Qualifications Master's Degree in Nursing New York State Registered Nurse Licensure 5+ years of experience in wound care Wound, Ostomy, Continence Nurse (WOCN) certificate or ability to become certified within 6-months of start date BLS and CPR certified Ability to establish and maintain effective working relationships with the interdisciplinary team Ability to function independently Or, a satisfactory equivalent combination of experience, education and training. 
Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. This information includes items such as observable physical behavioral changes, fatigue levels, and general behaviors and responses to routine treatments, such as foot soaks. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Job Description The Department of Pediatrics at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner, Outpatient Gastroenterology Infusion Center. The successful candidate will: Provide advanced care, including administering intravenous medications, fluids and nutrients to children to treat chronic illnesses, infections, or for wellness purposes like dehydration and fatigue. Demonstrate responsibility for patient management assessing and educating patients and families, and monitoring patients before, during and after therapy. Oversee pediatric infusions, providing a higher level of care and diagnostic capabilities, including ordering necessary lab tests and adjusting treatment plans. Work both independently and under physician supervision, to create treatment plans, start infusions, monitor for reactions, and educate families. Perform patient assessments, manage medications, order diagnostics, and assist with procedures like endoscopies and colonoscopies. Provide assistance in coordinating, scheduling of the outpatient procedures and prior authorizations for infusions. Coordinate patient care for infusions and therapy. Document all care provided in the patient's medical record; completes all required patient care documentation and submit all documents to the office. Develop further and manage patient care panel with participation in managed care programs, and other direct patient care programs as applicable. Participate in and provide departmental education activities to medical students, residents, fellows and allied health professional students. Participate in hospital and departmental health fairs, community education events and speak to community groups when requested.
