JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Job Title/Position: Registered Nurse 11. Assists the patient and family/caregiver and other team members in providing continuity of care. 7. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 8. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. Trauma and Emergency General Surgery Advanced Practice Provider (APP) Position Overview: The Trauma and Emergency General Surgery Advanced Practice Provider (APP) delivers direct patient care consistent with the APP’s education, experience, and certification, in collaboration with the attending Trauma and Emergency General Surgery physicians. This includes the diagnosis, management, and referral of health care issues specific to Trauma and Emergency General Surgery (EGS). The APP will complete pre- and post-surgical management and recognize options for non-surgical management of trauma, EGS, and critically ill patients. Key Responsibilities Collaborate with Trauma/EGS attending physicians, general surgery residents, and APPs to complete daily rounds on the trauma and surgical floors and in the intensive care unit (ICU). Respond to Emergency Department (ED) consults for EGS, collecting data and interpreting laboratory and imaging results to develop and document assessments and care plans. Perform bedside procedures for both surgical and non-surgical patients Work with the team to complete timely documentation and carry out care plans effectively. Partner with Trauma/EGS APPs, Trauma/EGS attendings, surgical residents, nursing, physical, occupational and speech therapies, social work and case management to develop disposition plans with a multidisciplinary team approach. Respond to trauma cases in the resuscitation bay, perform trauma survey assessments, and stabilize high-acuity patients in accordance with Advanced Trauma Life Support (ATLS) guidelines. Delivers direct patient care consistent with the advanced practice registered nurse’s education, experience and certification, in collaboration with a physician, which includes diagnosis, management and/or referral of health care issues. Location: METROHEALTH MEDICAL CENTER Biweekly Hours: 80.00 Shift: 8:00 AM TO 5:00 PM The Department of Emergency Medicine of the MetroHealth System, a principal teaching hospital of the Case Western Reserve University (CWRU) School of Medicine, is seeking Advance Practice Registered Nurse to join our growing team at our Life Flight team. Delivers direct patient care consistent with the advanced practice registered nurse’s education, experience and certification, in collaboration with a physician, which includes diagnosis, management and/or referral of health care issues. Promotes consumer, family, and community health through preventative health care actions and through patient advocacy. The advanced practice registered nurse may also manage complex illnesses and/or deliver specialty care. Upholds the mission, vision, values, and customer service standards of The MetroHealth System. Metro Life Flight: Responsible for advanced emergency and critical care patient management during air and ground transport of the critically ill or injured patient from the scene or outlying sites to participating secondary and tertiary care institutions. Provides intensive care monitoring, critical care intervention and advanced surgical skill procedures to critically ill and injured patients. This includes the management of high-risk obstetrical, medical, surgical, respiratory and cardiovascular patients. The specialized training of the Flight Nurse Practitioner allows for the provision of care to critically ill and injured patients through the use of ventilatory support, continuous cardiac and hemodynamic monitoring, including intra-aortic balloon pump. Assures availability via provided hospital pager for programmatic needs including but not limited to: patient care, specialty transports, and other administrative direction. Performs a prehospital scene survey to ensure safety of patient and crewmembers and coordinates patient triage in the pre-hospital environment as needed. Evaluates the care of patients within the scope of flight nurse practitioner utilizing Metro Life Flight protocols/guidelines and physician collaboration and consultation. Maintains current knowledge of transport medicine standards and responsibilities. Attends a minimum of 60% of scheduled staff meetings and documents meeting minutes have been read if not in attendance. Attends a minimum of 50% case review conferences. Attends minimum of two surgical skills lab a year. Attends Operating Room or clinical experience if privilege requirements have not been met. Perform techniques and medical procedures, via human patient simulators, as required to maintain skill levels.
