Observes the resident taking the medication, notes effects including any unexpected or adverse effects, correctly documents the required information in the residents’ medication administration records, communicates any concerns or observations with the Wellness Director, Wellness Coordinator or designated personnel. The Medication Aide also supports residents with activities of daily living including personal hygiene, socialization, cognition, and physical health. Critical Success Factors Successful completion of medication assistance training in compliance with the state’s regulatory requirements and facility requirements. Performs various clinical support tasks under clinical supervision within the organization's established scope of practice guidelines to ensure efficient patient flow. Prepare and rooms the patient for exam by obtaining vital signs and gathering/documenting/updating pertinent health information (i.e. chief complaint, allergies, and/or medications). Perform laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions/infection control guidelines. Assist the physician/advance practice provider (APP) with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc. Administer routine medications, under physician/APP orders, which may include but are not limited to immunizations, antibiotics, vitamins and topical agents. Nurse Practitioner As the Nurse Practitioner you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents. You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. As a Nurse Practitioner, you will provide types of care, including but not limited to: Be comfortable and proficient with documentation and CPT coding Evaluation of new-admission visits Coordination of discharge visits Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition Advanced Care Planning Other Duties & Responsibilities Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease Prescribe all necessary medications and treatments Perform comprehensive physical examinations of patients Develop and implement patient management policies and procedures Facilitate referrals to other healthcare professionals and medical facilities Communicate with attending physicians and/or specialists regarding patient care Be comfortable and proficient with documentation and CPT coding Requirements Licensure / Certification / Registration: Advanced Practice Licensure (APN) with an applicable state license Must have an active license and DEA # Previous experience in nursing or other medical fields Minimum one-year experience in a Skilled Nursing setting (preferred) Ability to build rapport with patients and their loved ones Ability to thrive in a fast-paced environment Excellent written and verbal communication skills Strong leadership experience with emphasis on teamwork in a healthcare setting Education Master’s degree required
