Job Description: Clinical Hospice Liaison Position Title: Clinical Hospice Liaison Position Summary: The Clinical Hospice Liaison serves as the clinical expert in the pre-admission process, assessing potential patients for hospice eligibility and coordinating a smooth and compassionate transition to hospice care.
