What You Will Do: Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC. Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care. Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions Submits clear and appropriate documentation in a timely manner and documents patient education. Communicates effectively to physicians, supervisors, and co-workers in a timely manner. Promotes continuity of care with appropriate admissions, transfers, and charges. Other duties as assigned. Demonstrates knowledge and correct use of all forms in regard to the day-to-day operation of the department. Assists physician with patient treatments. Offers support to patient and monitors patient throughout procedure and afterward. Completes dressing changes, suture and staple removal. Completes charge information accurately. Correctly identifies patient by name for any direct care procedures and/or treatment; checks identification of patient by use of their name band and registration sheet and assures that a patient is always banded with proper name. Always verifies with the patient the procedural laterality when appropriate Demonstrates an awareness of and understanding of the surgical patient's needs for comfort, positioning an safety, and follows through to provide these needs. Actively participates in interdepartmental meetings and in services. Reviews departmental minutes and seeks information through personal inquiry. Keeps self-informed of changes within department. Clinical Nursing Student – [Lorain Hospital] Job Summary: The Clinical Nursing Student provides direct patient care under the guidance of the Registered Nurse or designated healthcare professional in accordance with federal, state, and local regulations and within the policies, procedures, and guidelines of Bon Secours Mercy Health. The Clinical Nursing Student role is designed to provide clinical experience and exposure through both job shadowing of the Registered Nurse (RN) and provision of care appropriate to scope and at the delegation of the RN. Essential Functions: Under direction of the RN, supports the nurse in the application of the nursing process. Works to develop critical thinking skills through collaboration with the RN to ensure quality patient care delivery Seeks out educational opportunities related to nursing practice and critical thinking Provides basic patient care to include, but not limited to, care and comfort, record vital signs, personal care and hygiene, and mobility, including unit based specialty duties. Acts as liaison between patient and nurse to report changes or concerns to nurse Provides high level customer service to all patients, patient's family, visitors and all employees Provides accurate, precise, timely documentation when applicable for patient care Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving Performs delegated tasks once competency has been validated Participates in patient data collection and documentation (i.e. vital signs, intake/output, change in patient condition) Performs basic patient hygiene and care related to activities of daily living - assisting with ambulation and patient mobility, feeding, hydration and nutrition and bowel and bladder elimination Utilizes clinical technology as applicable Escalates concerns appropriately through appropriate chain of command Education: HS/GED (required) Currently enrolled in a nursing program and has completed one clinical nursing course and one clinical rotation (required) Required Licensing & Certifications: BLS (preferred) Experience: Recent experience in acute care and/or long term care facility (preferred) Skills & Abilities: Ability to clearly communicate to other members of the healthcare team Ability to understand and follow directions of healthcare team Knowledge of patient and healthcare safety standards and regulations (i.e. falls, skin breakdown, suicide prevention, environment of care) Infection prevention and transmission-based precautions Team dynamics Compassionate, relationship-based approach in care activities Accountability for completion of assigned tasks Critical thinking Advanced problem solving Understanding of nursing process Professionalism Training: Must have completed First Clinical Rotation as a Nursing Student EPIC Electronic Health Record (EHR) training (preferred) This is a PRN Position. Position Summary Performs professional nursing care and services for assigned physician(s) or department(s) under the general policies and procedures established by FPG. Under supervision of the physician(s), participates in providing health promotion, health maintenance, and management of illnesses. Responsible for assisting physicians with patient care and provides information to patients so they may fully utilize and benefit from clinic services. Responsible for performing a variety of clinical duties including but not limited to: monitoring patient flow, obtaining and documenting patient vitals, document current medications, chief complaint, performing technical laboratory functions including phlebotomy/EKG/stress testing, monitor/track/charge medical equipment, scheduling procedures and tests, and medical scribe duties. What you will need: Must be a graduate of an accredited program of practical nursing with current licensure in the state of Ohio. OR must be a High school graduate. Medical Assistant certification required. Must have the ability to perform as a medical scribe. Prior experience in a medical office/urgent care setting preferred. Current CPR certification. Must be knowledgeable of technical application of nursing process. Must possess critical thinking skills, including skill in exercising initiative, judgement, problem solving, and decision making. Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy.
