Department/Position Overview: Sentara Leigh Hospital Intensive Care Unit, AACN Gold Beacon Award recipient, is comprised of 20 licensed critical care beds and has an average daily census of 18 patients. Staffing ratios vary and can be 1:1 to 1:3 with 1:2 being the usual assignment. The ICU RN’s are required to have a wide array of knowledge, skill and ability to competently and safely provide critical care to our patients. Frequent ongoing education, new skill acquisition and skill validation are provided and supported by an experienced ICU CNS for new and experienced ICU nurses. This unit welcomes experienced nurses moving into this specialty. The 11-week Transition to Practice program focuses on the orientation transition experience with intentional knowledge support and skill acquisition for practice in IMC areas. ICU staff are supported by an interdisciplinary care team which in part includes unit based respiratory, physical, speech and occupational therapists. Interpreter, nutrition, palliative, chaplaincy and ethical services are readily available as well. Our ICU has onsite and eICU pulmonary critical care intensivists and their team of physician extenders 24/7. Physician lead, nurse driven interdisciplinary rounds are held daily for continuity of care and to address nursing’s concerns or questions. It is a team effort to heal wounds and to not create new wounds. -When wounds become worsened, the treatment should be changed within a 2-week period with the input from the Wound physician and PCP at facility. -The following preventative items should be completed for residents at risk of developing Center Acquired Pressure Ulcers: Orders for T&P, Preventive barrier cream for all residents (Using a barrier cream with Zinc is beneficial in findings), Pressure relieving device to chairs If resident has a stage 4 they need to be on a Roho or something comparable). Every resident in the facility should have a gel cushion at minimum to their wheelchair/Geri chair. Braden assessments completed with all new admissions and re-admissions, quarterly and with any change in condition of the resident. If the Braden scores lower than a specialty mattress should be applied. At risk care plans should be placed for all resident because everyone is at risk. It is a team effort to heal wounds and to not create new wounds. -When wounds become worsened, the treatment should be changed within a 2-week period with the input from the Wound physician and PCP at facility. -The following preventative items should be completed for residents at risk of developing Center Acquired Pressure Ulcers: Orders for T&P, Preventive barrier cream for all residents (Using a barrier cream with Zinc is beneficial in findings), Pressure relieving device to chairs If resident has a stage 4 they need to be on a Roho or something comparable). Every resident in the facility should have a gel cushion at minimum to their wheelchair/Geri chair. Braden assessments completed with all new admissions and re-admissions, quarterly and with any change in condition of the resident. If the Braden scores lower than a specialty mattress should be applied. At risk care plans should be placed for all resident because everyone is at risk. Rounds by UM’s/charge nurses and wound nurse to be performed to ensure that residents are positions appropriately, T&P q 2 hours and prn in bed and wheelchairs/Geri chairs, heels elevated and no skin to skin contact. 1. Administer medications within the scope of practice of the R.N. or L.P.N. Licensure. Maintain a current report of narcotics received and used. 7. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each resident that reflect his/her condition and progress. 8. Licensure, Certification, or Registration Requirements for Hire: Clin I, Clin II, Clin III: Current RN licensure in Virginia or eligible or compact state Licensure, Certification, or Registration Requirements for continued employment: Clin I, Clin II, Clin III: Current RN licensure in Virginia, AHA BLS HCP Certification or equivalent Experience REQUIRED: Clin I: N/A Clin II: 12 to 24 months RN experience Clin III: 3 years of clinical experience Experience PREFERRED: Clin I: Experience as an LPN, nursing assistant or care partner in either an acute care setting or as applicable to area of practice Clin II, Clin III: Academic Health Care Experience; Experience with similar patient population in either an acute care setting or as applicable to area of practice Education/training REQUIRED: Clin I, Clin II, Clin III: Diploma or Associates Degree from a professional nursing program recognized and/or approved by the corresponding state’s Board of Nursing. All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an approved RN to BSN program within two years of their start date and to complete the program within five years of their start date unless grandfathered under previous requirements.
