The Nurse Care Manager provides assessments, evaluation, triage, teaching, medication induction, stabilization, and monitoring of patients in the grant funded opioid treatment (OTP) program. He/she collaborates with OTP team members and outside referral sites, works with medical director and DATA-waived providers when treating patients, and with other addiction and mental health programs and primary care providers in assisting patients in recovery and supporting their referral needs. The Nurse Care Manager operates independently, working cooperatively and collaboratively with patient, families, health care providers, payers, managers, and network partners. The Nurse Care Manager provides nursing diagnosis, counseling, education, and follow-up to patients served by these programs and makes assessments of potentially eligible persons and follow-up care as indicated by their assessment.
