OVERVIEW: Provide quality nursing care to residents in a long-term care environment which promotes their rights, dignity, freedom of choice, and their individuality under the supervision of a Registered Nurse. RESPONSIBILITIES: Provide individualized attention which encourages each resident’s ability to maintain or attain the highest practical physical, mental, and psychosocial wellbeing Knowledgeable of the individualized care plan for residents and provide support to the resident according to the care plan. Contribute to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents’ needs and preferences Attend to the individual needs of residents which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals’ care requirements, and scope of practice Maintain the comfort, privacy, and dignity of each resident in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring environment Fully understand all aspects of residents’ rights, including the right to be free of restraints and free of abuse Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents’ rights Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the charge nurse such as lighting or equipment problems Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items Observe residents for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel Provide care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin, etc. Answer residents’ call bells promptly and courteously Perform various tasks assigned by the charge nurse including vital signs, residents’ weights, applying creams/ointments, collecting specimens, etc. Assist with orienting residents and their families to the nursing home upon admission and to the unit when transfers occur Assist and escort residents for appointments such as beauty shop, activities, church services, etc. Complete certified nursing assistant records while using proper coding of activities of Daily Living (ADLs) documenting care provided or other information in keeping with department policies Follow prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials Support an atmosphere which promotes a homelike environment Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: A high school diploma or equivalent; successful completion of a state-approved Nursing Assistant Certification course or other education which meets state certification requirements Must hold current Nursing Assistant Certifications in the state in which they are practicing Must be knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment. Current CPR (American Heart Association) SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities. WORK ENVIRONMENT: This job operates in a health care setting. This role requires regular walking to various locations around the care center. Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment. Incumbent may be required to work extended periods of time at a video display terminal. Incumbent may experience traumatic situations, including psychiatric, and deceased patients. Provides clear communication across the continuum to improve patient outcomes. Assures quality, cost-efficient care for an identified patient population through the continuum of care. Develops and coordinates efforts to reduce clinical resource utilization by working closely with physicians, nursing staff and other hospital personnel. Reviews medical records for appropriateness of admissions, continued stay and level of care changes against criteria of severity of illness and illness of service to ensure optimal reimbursement to the hospital.

This Temporary position will be in Bayonne University Hospital. This will be contracted for 13 weeks and will be reviewed on a weekly basis dependent on need. You will not be considered if you do not have OR experience. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Providing assignments and schedules for nursing and paraprofessional staff - Utilizing Automated Student Health Record (ASHR) to generate reports - Conducting in-service trainings and planning staff development activities - Participating in recruitment and orientation of new staff - Analyzing data collected in OSH management reports, i.e. walk ins, case management and special projects - Participating in disciplinary activities as needed - Conducting site visits to school to provide clinical supervision of nurses, advisors, and assistants - Collaborating with the OSH team (Supervising Medical Doctors, Vision team, Mental health team, etc.) to plan initiatives within their assigned area - Monitoring inventory of medical supplies and equipment within a designed area - Serving on emergency teams which are on call for all future bioterrorism attacks within the City of New York as well as all emergencies as directed by the Commissioner of the Department of Health and Mental Hygiene. - Supervising Public Health Nurse Level 3 OSH is an important role to ensure the health and well-being of our students, improve the efficiency of health services, and directly contribute to a healthier safer school environment for all students. Position Overview: This role is based at a healthcare facility in the Bronx, supporting admissions, care planning, and overall nursing department operations while ensuring high-quality resident care. Admissions experience is a must! Duties & Responsibilities: Conduct admission assessments for new residents and hospital returns; initiate care plans. Review PRI documentation and coordinate all aspects of the admission process with the interdisciplinary team. Assist in the development, implementation, and oversight of nursing policies, procedures, and department operations in collaboration with the Director of Nursing (DON). Schedule and assign nursing staff to effectively meet resident care needs. Foster a compassionate environment by supporting residents and their families. Provide information and respond to questions from residents, families, and staff. Review and revise resident care plans based on changes in condition. Participate in discharge planning as needed. Complete MDS, charting, and all required documentation in accordance with policies and procedures. Audit resident and departmental records to ensure compliance with facility standards and regulatory requirements. Maintain a safe, clean, and compliant working environment by adhering to established guidelines. Ensure confidentiality and proper handling of resident information in accordance with HIPAA and corporate policies. Maintain nursing supply inventory and related operational needs. Supervise or direct resident care as assigned. Requirements/Qualifications: Active, unrestricted New York State Registered Nurse (RN) license. Long-term care (LTC) experience required; admissions experience needed. Strong clinical assessment and critical thinking skills. Ability to make sound, independent decisions. Demonstrated leadership and supervisory abilities. Basic computer proficiency; familiarity with Microsoft Office (Excel, Word, Outlook) preferred. Excellent communication and interpersonal skills.
