Evergreen Commons is looking to hire a full-time, Monday-Friday day position, dynamic MDS nurse to work at our skilled nursing facility. This role includes flexible hours, mostly remote with 1-2 days a week on site. Your duties will include facilitating the MDS process and assessments of residents as necessary. You will also work with the medical and interdisciplinary team to develop patient care plans. MDS Nurse Responsibilities: Assessing and monitoring patients' health in accordance with medical laws, regulations, and guidelines. Collecting patient data and organizing the information into reports for further research. Communicating with health care teams to create applicable health care plans for patients. Assisting and alerting patients with any notice of non-medical coverage updates MDS Nurse Requirements: NYS RN license. Solid knowledge of MDS processes such as the Resident Assessment Instruments (RAI). Excellent organizational and multitasking skills. The ability to make independent judgments utilizing initiative, discretion, and the nursing process, as needed.
