These patients receive care for a variety of life-threatening medical and surgical conditions. Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. This AHP position may perform delegated functions as appropriate to their "scope of practice". In general, the "scope of practice" includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per approved standardized procedures as identified by the list available on the Medical Staff Office website. The CIDP approved standardized procedures are unique to each person and are unlikely to include ALL SPs on the list. This position may provide patient care in approved age groups and UCSF Health licensed settings.
