Position Summary Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions. Provides clear explanations of hospice philosophy, services, and the admission process to ensure understanding and promote a smooth and supportive transition to hospice care. Working collaboratively with the interdisciplinary team, this role helps ensure that patients who are appropriate for hospice services are identified, informed, and efficiently connected to care. Education Graduate of an accredited school of practical nursing. Experience Completion of the Agency’s Orientation Program of the Valley Home Care. Skills Current and valid NJ license and CPR certification. Ability to use critical thinking skills to set priorities, effectively problem-solve, and perform work accurately with attention to details, often changing from one task to another w/o loss of efficiency or composure. Ability to handle frequent interruptions including emergencies and adapt to changes in workloads and schedules. Ability to function competently in stressful situations related to patient needs (e.g., working w/ patients w/acute, chronic, and complex disease processes, and those who are dying and work w/ patients of varied age groups. Ability to work cooperatively w/ members of the healthcare delivery team, patients, family members, and co-workers. Must have RN license to practice in New York, Reliable experience and BLS/CPR certification. Experience in a LTC/Nursing and rehab facility a plus. Full time and weekend shifts available. Responsibilities (including but not limited to) Complete and file required record keeping forms/charts upon the residents admission, transfer, and/or discharge Chart nurses notes in an informative, relevant, concise, and descriptive manner that reflects the care provided to the resident, as well as the residents response to care. Complete accident/incident reports as necessary.
