About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the West Valley (Phoenix) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the West Valley (Phoenix) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. We look forward to meeting you soon!​ Responsibilities Under the direction of the Chief Nursing Officer/Director of Nursing, Nurse Manager or Charge Nurse, the Registered Nurse (RN) is responsible for utilizing the nursing process while delivering, directing and supervising care to patients admitted to the nursing unit. The Registered Nurse plans, coordinates, and provides patient care to include interdisciplinary planning, discharge planning, patient and family teaching for post-hospitalization, and accessing community resources. May perform other related duties as assigned or requested, including Charge Nurse (ACLS certification required). Qualifications Education and Training: Holds a current licensure as a registered nurse (RN) in the state where the hospital resides, or compact licensure if state participates. Current BLS certification is required. Evidence of ACLS certification is required within 90 days of employment. BLS and ACLS certifications must be AHA approved within 90 days of employment. Current ACLS certification is required for Charge Nurses. Experience: One year of inpatient medical-surgical nursing experience preferred but not required.
