RN (Registered Nurse) OFFERING DAILY PAY!! We are looking for a talented and hard-working RN to join their ever-growing team. RN (Registered Nurse) Job Duties: Assists in defining, maintaining and interpreting standards of practice. Performs required skills (assessment, intervention) according to guidelines, policies and procedures. Participates in interdisciplinary committees which influence or determine policies affecting nursing practice. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on established guideline. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s and/or others under his/her supervision receive their resident reports, has their assignments and break times scheduled. Assist the L.P.N. in establishing routines which facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Nighttime responsibilities may include: Completing scheduled safety checks Monitoring the client’s condition Assisting with toileting and incontinence care Repositioning the client Providing medication reminders when authorized Responding immediately when the client needs assistance Completing approved nighttime housekeeping responsibilities Maintaining a clean and safe environment Preparing the client’s environment for the morning routine Documenting care and observations in real time Sleeping, napping, dozing, lying down to rest or closing your eyes for extended periods is strictly prohibited, even when the client is sleeping. Failure to remain awake and attentive may result in immediate removal from the assignment and additional corrective action. Physical Requirements Caregiving can be physically demanding. Applicants must be able to safely perform the essential responsibilities of an accepted assignment, with or without a reasonable accommodation. Clock in and out from the approved client location. Remain with the client until the scheduled end of the shift. Remain until properly relieved when continuous care is required. Never leave a client unattended when supervision is required. Never leave early without office authorization. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. This includes but is not limited to: · Recording Screens/Resident Needs Reviews, evaluations, daily treatment notes, discharge summaries and walking rounds in accordance with facility Policies and Procedures. · Recording treatment charges accurately and ensuring that documentation is consistent with billing data. · Writing specific, objective documentation that is outcome oriented and details the patient’s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals. 3 . Consistently demonstrates facility interdisciplinary approach to patient care. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Contributes to professional development of self. · Maintaining current professional status and licensure. · Adhering to professional standards of conduct and clinical code of ethics and practice. · Implementing in-services and training. · Maintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers. · Meeting continuing education requirements of regulatory agencies. 5. Aids in building and maintaining the facility’s reputation as the unparalleled respiratory therapy provider by contribution as a professional member of the staff. · This includes but is not limited to oversight of the following departmental operations: · Consistently demonstrating the facility’s core values by interacting with co-workers, patients and families, with all other persons involved with the patient’s care. · Consistently demonstrating professional standards as outlined in the facility’s Employee Handbook and as communicated by the facility’s management. · Contributing to a positive work team by sharing information, using problem solving methods and accepting new ideas, constructive criticism, or advice from others. · Maintaining close communications within the facility with the director of nursing, nurse managers, and admissions coordinator to assist in development of the respiratory therapy program. · Establishing and maintaining an effective rapport with the facility administrator, MDS, Rehab Director, DON, and medical director. o Demonstrating superior customer service through courtesy, consistent follow-up and positive communication. 6. Demonstrates safe working practices . This includes but is not limited to: Identifying and correcting safety hazards or notifying the facility or the admin so that safety hazards will be immediately remedied. Maintaining work areas in safe and orderly fashion. Maintaining equipment in safe working order by facilitation annual calibration. Using proper body mechanics during treatments of patients and during support activities. Using universal safety precautions. Wearing appropriate protective equipment.
