Assist with promoting a compassionate physical and psychosocial environment for the residents. Responsibilities of Certified Nursing Assistant (CNA): Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Requirements/Qualifications of Certified Nursing Assistant (CNA): Current Nursing Assistant certification from New York. Current/active CPR certification, preferred. At least one (1) year Skilled Nursing experience preferred. Effective verbal and written English communication skills. Highest level of professionalism with the ability to maintain confidentiality. Rutgers, The State University of New Jersey, is seeking a Part Time Licensed Practical Nurse for Northern State Prison located in Newark, NJ The primary purpose of the Licensed Practical Nurse position is to, in accordance with professional nursing standards, skillfully administer total patient care based on the individualized needs of assigned patients and families. Under the direction and supervision of a Registered Nurse, provides technical nursing care to assigned patients, performs treatments and procedures within established guidelines, and provides a safe and therapeutic environment. Among the key duties of this position are the following: * Participates in data collection related to the patients’ status and needs. * Provides input to Registered Nurse for incorporation into the plan of care by communicating information pertinent to the patient’s status. * Provides direct patient care within scope of practice to ensure implementation of the plan of care. * Informs and instructs patients/significant others on patient’s health/illness state and plan of care. * Assists patient/significant other in preparation for discharge and health maintenance in the home. * Documents all aspects of patient care.

Registered Nurse (RN) Risk Manager Requirements/Qualifications: RN with valid NY License and prefer two (2) years’ experience as DON, ADON, or MDS in a long term care center. Completion of post-secondary education and be able to read, write, and follow oral and written directions. Must be capable of maintaining regular attendance as requires. Related experience at a level necessary to accomplish this position. Must be able to adjust to a flexible schedule to include evenings, nights and weekend work hours.
