JOB SUMMARY: The Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education/experience. RESPONSIBILITIES: Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops the plan of care with patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient/family as appropriate per assessment and plan. Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Valid driver’s license Current certification in Basic Life Support for Health Care Professionals. Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Inventories, orders or re-stocks medical supplies as applicable. 5. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. 6. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. 7. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. 8. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. 9. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. 10. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). 11. May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. 12. May be required to perform proficient phlebotomy duties. 13. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. 14. May maintain, adjust and confirm patient office schedule. 15. May check and verify and update insurance information utilizing internal or web sources. 16.
