Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups. Act as a liaison with AccentCare Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families. Make inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse. Provide consultation on hospice and palliative care.
