They will coordinate the care of patients under the supervision of a Board-Certified Physician. In addition, the Family Nurse Practitioner acts as a member of the clinical team and works collaboratively with other departments in providing patient care. Essential Functions: Delivers direct primary care and preventative services to assigned children, adolescent, and adult patients. Develops therapeutic plans of care for prevention, management, and maintenance of good health for patients. Provides health education and counseling patients. Provides care for Acute and Chronic Illness and Urgent Care services, which includes assessment, diagnosis, treatment, and monitoring of illness. Works closely with multidisciplinary teams and instructs and assists other team members in management of patient care. Follows established practice standards and current evidence-based clinical practices and guidelines. Provides care related to prenatal/postnatal services and care for uncomplicated pregnancies, in conjunction with physician partners, as delineated and approved for in request for clinical privileges. Provides family planning counseling and services. Consults with physician partner and refers patients with more complex/dynamic problems to subspecialty care, as appropriate. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within the specified amount of time expected by the Clinical Director. Ensures that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment. Attends trainings, weekly Provider and Team meetings, and collaborates on practice, administrative, and quality improvement projects, as assigned by Clinical Director Provides clinical supervision and mentoring to Nurse Practitioner, Medical, and Physician Assistant students by mutual agreement with the Clinical Director. May work a fluctuating schedule, including days, evenings, weekend shifts, and holidays according to schedule approved by Clinical Director. Serves as a single point of contact for referring physicians, clients, caregivers, and human service organizations to provide resources and assistance with behavioral health care services offered within South Shore Health System and its partnering healthcare community. The BHN will provide brief interventions in Acute Care, primary care and other community settings, educate clients on the importance of MH services, and navigate clients to a long-term provider. Provide brief behavioral health interventions in the form of Individual, Family, or Group treatment. Possesses knowledge of substance abuse assessment, recovery principles, behavioral activation, and chronic disease self-management. 6. Complete intake of high-risk clients, working with the client, family and other members of the healthcare team as needed, to assess client’s behavioral health needs, physical health, support system, financial resources, and available community and government resources. Refer to crisis services as needed. 7. Determine with the client specific objectives, goals, and measures that meet the client’s needs and that have been identified through assessment. Utilize the needs assessment to find a mental health home for the client and provide warm hand-offs to long-term mental health providers. 8. Serve as an essential link between clients and all other care providers. Represents client/family by intervening, negotiating and promoting their concerns. Problems requiring advocacy may include social and health inequities or inadequate and non-existent hospital and/or community resources (i.e., insurance benefits, housing, transportation, etc.) 9. Position Responsibilities: 1. Recognize and respond to the needs of the patients and families 2. Recognize and communicate changes in the patient’s condition to the appropriate nursing team member 3. Adhere to Safe Patient Handling practices, i.e. maintain a no manual lift environment 4. Participate in Quality Improvement programs 5. Assist patients with all aspects of personal care as indicated in the care plans 6. Document pertinent information regarding care delivered in the designated areas of the patient’s electronic medical record 7. Report accidents and incidents as they occur 8. Report any observed or suspected patient abuse immediately 9. Observe and report skin integrity 10. Participate in daily report team and care planning meetings as needed 11. Be proficient in obtaining vital signs and other procedures such as temperature, pulse, respirations, blood pressure, weight, pulse oximetry and urine and stool specimens as directed 12. Follow infection control practices 13. Attend and participate in ongoing training and educational classes 14. Maintain confidentiality and privacy of the patient according to HIPPA regulations 15. Qualifications : Long term chronic care PCA Certified Nursing Assistant certification is preferred in good standing upon hire or current enrollment in a nursing school, having completed first clinical experience. RSU PCA: Certified Nursing Assistant certification is required in good standing upon hire and on-going.
