Responsibilities Interview’s patients to determine medical problem/condition and documents in chart (EMR) for physician, when necessary obtains and records patient’s vital signs including weight and assists physician with patient examination as needed and explains procedures and treatments to patient to gain cooperation, understanding and allay apprehension, maintains awareness of comfort and safety needs, manages lipids, protimes and phone triage of patient concerns. Follows up on physician orders and lab requests, obtains direction from physician as needed and informs patient of test results and any further treatment prescribed; provides explanation and education to patient; documents services performed in EMR for billing purposes, observes patient, records significant conditions and reactions, notifies supervisor or physician of patient’s condition and reaction to drugs, treatment, and significant incidents, and assists physician in clinical care of patient. Documents nursing history in EMR and physical assessment for assigned patients and initiates a patient education plan according to the individualized needs of the patient, as prescribed by the physician. Abstracts patient records via the EMR. May sort or do chart prep on occasion but not as a routine part of the position. Uses other EMR functions such as tasking, messaging, and disease management. Uses Centricity and other software as appropriate to support nursing functions. Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. Maintains crash cart and defibrillator. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. VAD Program Coordinator-RN - Main Advanced Heart Failure - Full Time - Days

Job Description Ventricular Assist Device (VAD) Patient Coordinator is a clinical expert registered nurse who applies clinical expertise in evidence-based practice. Applies concepts derived from management and communication theory, group dynamics and clinical judgment to implement the VAD program. Integrates care with multidisciplinary team (within the hospital, practice and in the community) to facilitate VAD patient care from pre implant thru discharge of care. Clinical Expert/Consultant : Demonstrates or supports implementations of nursing process for VAD/HF patients. Recognized advisor sought out for clinical expertise related to VAD/HF. Responsibilities Educator: Evaluates competency of clinical staff (inside and outside of the hospital), patients, family members, physicians, and community health resources related to the care of patients with VAD’s. Researcher: Functions as Study Coordinator or Co-Investigator for national investigational studies related to VAD and HF conducted at TCH. Utilizes and communicates applicable research to improve practice. Case Manager: Functions as case manager for all VAD patients from time of referral to implant through discharge with ongoing case management thereafter. VAD Patient Care Manager: Manages equipment, supplies, policies and procedures related to the VAD Program as assigned. Staff on this unit undergo special training and education regarding non-invasive positive pressure ventilation (Bi-PAP, C-PAP, and A-PAP). Heart & Vascular Medical Stepdown (Heart Failure): The Heart Failure/7 South consists of 28 private medical stepdown beds in a family-centered environment. In addition to caring for heart failure patients, the unit specializes in the care of patients diagnosed with MI, cardiac arrhythmias, syncope and cardiomyopathy as well as the recovery of patients following ablation, pacer placement/ICD and PTA. Medical services are offered to all types of patient populations requiring cardiac monitoring, including renal, endocrine, pulmonary, oncologic, general medicine and geriatric. Other services provided within this unit include titration of multiple intravenous medications and conscious sedation for cardioversions. In addition, a select group of nurses complete special training to provide ultrafiltration for decompensated heart failure patients and left ventricular assist device (LVAD). All nurses are ACLS certified and have completed an EKG interpretation course. This is an excellent unit for nurses interested in transitioning to an ICU setting. Cardiovascular Stepdown (CVSU): The Cardiovascular Step-Down Unit is a 32-bed, surgical step-down/progressive care unit. The primary focus of this unit is cardiothoracic surgery and interventional cardiology. Patient populations include LVAD, innovative valve replacement, pectus excavatum and post STEMI. Nursing care includes management of cardiac arrhythmias, titration of multiple intravenous medications and conscious sedation for cardioversions. All nurses on this unit are ACLS certified, have successfully completed an ECG interpretation course and follow the AACN Progressive Care Guidelines. Nursing staff possess and maintain specialty skills including chest tube removals, femoral arterial line removals, complex external pacing, ACT and iSTAT point of care testing and much more. CVSU staff includes a clinical manager, clinical assistant managers, clinical educator, registered nurses, patient care assistants, health unit coordinators, cardiac monitor readers and exercise physiologists. Specialty certification and minimum BSN education for staff on this unit are highly encouraged.
