Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1. POSITION SUMMARY The Hospice Program Director is responsible for the overall direction of hospice clinical services. Maintains the quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, implementation and evaluation of the hospice services provided. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Coordinates and oversees all direct and indirect patient services provided by the clinical organization personnel. Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards. Perform chart audits. Gather data for QAPI and participate in QAPI activities. Appraise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff. Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff. Oversees the maintenance of patient clinical records and assures compliance with local, state and federal laws as well as ACHC standards of care. Assist with the orientation of new staff. See patients in the home setting as needed. Participate in audits, IDG meetings and QAPI. Maintains adequate and appropriate Medical Supplies. Must have a minimum of 2 years of nursing experience in home hospice or skilled home care. (hospice preferred) At least one (1) year of supervisory experience. Ability to market and deal tactfully with customers and the community. Knowledge of business management, governmental regulations and ACHC standards. The ability to communicate well, both verbally and in writing.
