The Registered Nurse (RN) Urgent Care delivers nursing care for patients across their life span, and through the nursing process, assesses plans, implements, and documents the effects of care. Within the urgent care setting care will be provided to occupational health patients (physicals, drug screens, injury care etc.,) and patients that arrive with an acute illness/injury. This individual will work cooperatively with ancillary, nursing and other patient-team personnel to maintain standards for professional nursing practice. These nursing functions of the RN are carried out according to the Pennsylvania Nurse Practice Act, Hospital policy, the hospital Mission Statement, and the Philosophy of the Department of Nursing. JOB DUTIES AND RESPONSIBILITIES: Demonstrates the ability to perform the essential functions of the Registered Nurse. Provide essential medical care to both urgent care patients and occupational health patients Demonstrates knowledge of procedure done in this area and is able to assist physician as indicated. Implements, evaluates, and documents patient care interventions and effectiveness in accordance with care plan. Demonstrates an awareness of unit budget. Functions as a professional role model and resource person providing guidance to co-workers and students. Takes active role in unit-based Performance Improvement. Participates in identifying unit needs and supports unit goals and objectives. Maintains responsibility for own professional development through active participation in in-services and/or continuing education programs. Develops, evaluates, and adjusts current and future staffing based upon patient care needs; initiates adjustments as needed. Assigns responsibility for patient care with the unit-based team. Facilitates staff education activities (i.e. orientation, competency, skill reviews, mandatory in-servicing). Provides input to annual performance reviews of assigned staff. Conducts customer service activities and handling of complaints – patients, families, staff or physician. Assists with management functions on unit. Manages daily operations within budget parameters. Performs in depth, systematic assessment of all assigned patients. Formulates collaborative plans of care and identifies expected patient outcomes. Implements, evaluates, and documents patient care interventions and effectiveness in accordance with care plan. Organizes, coordinates, and prioritizes patient care consistently utilizing available resources. PHYSICIAL AND SENSORY REQUIREMENTS: Sit up to 2 hours per day; 1 hour at a time. Stand for up to 8 hours per day; 4 hours at a time. Walk 6 hours per day; 10 minutes at a time. Consistently life, push and carry objects up to 10 lbs. Frequently life, carry and push objects up to 75 lbs. Transport patients weighing up to 400 lbs. via wheelchair, bed and/or stretcher with assistance when appropriate. Consistently pull up to 10 lbs. Frequently pull up to 250 lbs. with assistance when appropriate. Frequently stoop, bend, crouch, kneel and climb. Frequently reach above shoulder level. Occasional crawling. Must be able to perceive attributes of an object through touch. Frequently finger and handle objects. Occasionally firmly grasp, twist, turn objects with hands and fingers. Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral vision, depth perception and visual monotony (e.g. computer screen). EDUCATION: Registered Nurse with current license to practice in the state of Pennsylvania. TRAINING AND EXPERIENCE: Successful completion of BLS. Reviews prior claims to address potential impact on current case management and eligibility. Assessments include the member’s level of work capacity and related restrictions/limitations. Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality. Consults with supervisor and others in overcoming barriers in meeting goals and objectives, presents cases at case conferences to benefit overall medical management. Utilizes case management processes in compliance with regulatory and company policies and procedures. Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation. The position focuses on managing incoming referrals for nursing facility transitions, maintaining a strong on‑site presence, and coordinating safe, timely discharges that promote independence and quality of life. The individual will be embedded in assigned nursing facilities 2–3 days or more per week, based on member needs and acuity, building trusted relationships with facility staff and interdisciplinary teams. Responsibilities include assessing member readiness to return home, facilitating Interdisciplinary Team (IDT) meetings, and developing comprehensive, member‑centered transition plans of care. Rather than carrying a traditional caseload, this role is outcome‑focused, with a monthly goal of successfully transitioning three members from nursing facilities back to the community. The position also requires community follow‑up, meeting members face‑to‑face after discharge to complete a NJ Choice Change in Condition assessment within 10 business days of discharge. The Clinical Triage Specialist (CTS) (RN) - Access Center will compassionately deliver an exceptional patient experience and provide clinical support to CTS-MA team members by serving as a clinical resource. The CTS-RN is responsible for using nursing judgment in answering/returning patient calls related to direct care provided by the practices. When appropriate, the caller’s symptoms will be assessed and triaged using approved nursing protocols and guidelines to assist in obtaining the appropriate level of care and/or self-care advice. JOB DUTIES AND RESPONSIBILITIES: Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed. Verifies patient demographic information and accurately enters the updated information into electronic health record. Serves as an escalation point for clinical patient issues and other POD team members requiring clinical support, and provides clinical advice based on clinical protocols and procedures. Manages and responds to escalated electronic patient messages whenever not answering inbound patient calls and uses clinical judgment to prioritize and accommodate patients. Creates a positive patient experience at every encounter, attempting to independently resolve any issues or concerns of the patient at the time of the phone call, within the scope of the role. Consistently meets productivity, schedule adherence, and quality standards as set by the Access Center. Utilizes all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate medical facility or personnel. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Partners with other Access Center teams/PODs and respective practice clinical team on behalf of the patient to assist with clinical concerns, medication refills, or scheduling appointments. Other duties as assigned. EDUCATION: Graduate of an accredited nursing program. Active Registered Nurse licensure in the state of Pennsylvania and New Jersey or other nursing compact state and other states as deemed necessary by state law. Position: Travel - Advanced Practice Provider - North New Jersey The Monogram Advanced Practice Provider is a Nurse Practitioner is a critical role that provides access to comprehensive care to underserved patients within their assigned territory. The Advanced Practice Provider will be responsible for delivery of personalized compassionate medical care to patients primarily with CKD through ESRD. The Advanced Practice Provider will be responsible for caring for patients, maintaining accurate patient records and scheduling and administering follow-up appointments to patients as required across multiple markets. The successful candidate will work as a team with our field-based nurses, community health workers, and physicians to assist in delivering premium care to every patient. Primary duties include patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. Advanced Practice Providers perform the important task of educating patients about preventative care and prescribed treatments. He/she may also conduct physicals, order tests, prescribe medications, and serve as a coordinator with the patient's primary healthcare provider and their specialists.
