Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Tasks: Reviews all cases within 24 – 48 hours or the next business day of admission/bed placement and each day throughout the stay to facilitate care progression to establish an anticipated length of stay and transition planning needs. Collaborates with the medical team to formulate a treatment plan to include care transitions and promote patient flow. Completes an initial assessment of all admissions/observation patients to identify barriers that impact the length of stay and discharge planning. Job Summary: Essential Duties and Responsibilities Maintains competencies on Mother/Baby, GYN, Pedi and basic Med-Surg. Maintains competencies in either L&D or ‘sick baby’ management (L&D track and/or Nursery track). Once training and competencies are completed in the RN’s designated ‘track’, and the Clinical Director and Clinical Resource Nurse deem appropriate, the RN may train to the other track if desired—both sets of competencies must be maintained. Renders professional nursing care to patients within an assigned unit of the hospital, in support of medical care as directed by medical staff and pursuant to objectives and policies of the hospital Compliant in completion of all unit specific competencies and requirements required to care for all patients/patient types with the department. Involved in quality improvement planning and processes for the area of practice Clinical documentation and care plans completed comprehensively for all patients assigned in a timely manner Seeks feedback regarding practice and takes actions as appropriate.
