In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Nurse Practitioner experience of at least one year preferred Licensure: New York State License with current registration as a registered nurse. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. Monitor surgical wounds, drains, vascular status, laboratory results, and overall recovery. Recognize and appropriately manage post-operative complications in collaboration with the vascular surgeon. Manage pain, medications, fluids, and other aspects of routine inpatient care within the scope of practice. Provide patient and family education regarding treatment plans, recovery, and discharge needs. 3. Inpatient/Floor Responsibilities Perform daily rounds with the vascular surgery team. Write daily progress notes, admission notes, consult notes, discharge summaries, and other required clinical documentation. Place appropriate orders and manage routine inpatient needs. Coordinate consultations with other medical and surgical specialties as needed. Communicate with nursing, case management, physical therapy, social work, and other members of the multidisciplinary team. Qualifications Graduate of an accredited Physician Assistant or Nurse Practitioner program. Current New York State licensure as a PA or NP. Description The Nurse Manager (NM) is responsible for the management of nursing practice and operations on a designated unit/service setting such as inpatient, procedural, emergency department or ambulatory care. Description P osition Title: Licensed Practical Nurse-Greenwich Village Clinic Nursing-Mount Sinai Hospital-Full Time-Days Position Summary: The practice of nursing as a Licensed Practical Nurse is defined as performing tasks & responsibilities within the scope of supportive & restorative care; medication administration and specialized tasks under the direction of a Registered Professional Nurse of licensed or otherwise legally authorized physician or dentist. Responsibilities A. Clinical/Technical/Service Participates in the application of the nursing process to an assigned group of patients. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Qualifications Education and Experience requirements: Completion of educational program for licensure as a LPN. Successful completion of required New York State Department of Health LPN IV Therapy Initial Program for the demonstration of competence in the performance of intravenous therapy/blood administration procedures as part of Centralized Orientation. Description Palliative Care at Home provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurses responsible for providing high quality evidence based complex, chronic care management and palliative care in patients’ homes. Responsibilities • Work as member of interdisciplinary team assessing and addressing the complex care needs of patients and families facing serious illness. • Visits with an caseload of patients and families in their homes to complete guided assessments, support medical management, address complex symptoms, and coordinate care. • Provides supportive individual and family counselling to reduce anxiety/stress and symptoms precipitated by advanced illness and hospitalizations. • Engages patients and caregivers about goals of care, reviews advance directives, and collaborately develop treatment plans based on goals • Monitor and coordinate care plans in conjunction with the rest of the clinical care team. • Evaluates medical needs for caseload of patients, provide them direct in home care • Facilitate visits by the providers remotely as needed • Communicate plan of care to consulting clinicians and PCaH interdisciplinary teams • Educates patient/family on trajectory of illness, use of medications, assesses disease understanding • Participate in weekly IDT meeting with care team • Documentation as required • Performs other related duties as assigned Qualifications Required: Graduation from an accredited Nursing program.
