Summary The Ambulatory RN 2 identifies patterns of patient responses and can use past experiences to implement solutions for current situations. The Ambulatory RN 2 is a competent nurse who has mastered technical skills and utilizes evidence based interventions. The Ambulatory RN 2 continues to collaborate with other members of the health care team when the need for assistance is identified. Other Information Required Job Qualifications: Current license and credentials by the state they are assigned. At least one year of nursing experience required. Earned Nursing Degree at the Baccalaureate level or higher for those hired after January 1, 2014. All transfers to Rush University Medical Center or Rush University Medical Group from other Rush entities must have an earned nursing degree at the baccalaureate level or higher prior to the transfer. Chief Nursing Officer must provide approval for any RNs with ADN degree hired after 10/22/2020, based upon unit need. ADN RNs hired after 10/22/2020 must show continued enrolment towards a BSN degree with a signature of contract stipulating a BSN degree must be completed within 3 years of hire, Failure to do so will result in termination. ADN RNS are not eligible for leveling to RN3. Current CPR Certification. Analytical ability. Communication skills in order to lead and teach. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Be the Best Hospice Aide You Can Be If you meet these qualifications, we would love to meet you: Certified by the state as a nurse’s aide, with completion of required training and a passing score of 85% or higher on the agency’s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD). CPR certification required for direct patient care in Georgia and Washington. Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals. Responsibilities: Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance). Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care. Document care accurately and timely to reflect the evolving plan of care, including interventions, goals, and patient needs. Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols.
