The LPN Team Lead – Community Health provides day‑to‑day clinical oversight, coaching, and operational coordination for Community Health Workers (CHWs) and other frontline staff delivering community‑based services. This role blends hands‑on clinical practice with leadership responsibilities, ensuring safe, high‑quality care in home, community, and outreach settings. The LPN Team Lead supports access to health, chronic disease management, social determinants of health interventions, and care navigation. This role serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information.
