Description Job Description PeaceHealth is seeking an LPN Home & Community for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $28.73 – $43.09. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Hiring Bonus may be available! Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties. Essential Functions Implements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan. Provides direct patient care in patient homes including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains. Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed. Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting. Documents patient care in health record according to department standards. Delegates, prioritizes, requests and accepts guidance and assistance from others to assure delivery of care in a safe and timely manner. Ensures own professional effectiveness through continued education and professional development. Assists in the transporting of patients, specimens, supplies and equipment and in the maintenance and stocking of supplies and equipment to patient homes. May triage patient phone calls, counsel patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs. Teaches the patient and caregivers about disease management, in accordance with the plan of care. Performs other duties as assigned. Qualifications Education Accredited State Board of Nursing LPN Program Required: Experience Minimum of 1 year Preferred: clinical experience as an LPN and Preferred: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs Credentials Required: Upon Hire Licensed Practical Nurse - Oregon and Required: Upon Hire Basic Life Support and Required: Upon Hire Driver's License and proof of insurance Skills Basic computer skills required including familiarity with MS Office applications. (Preferred) Must have cooperative working and interpersonal skills and the ability to work effectively with a wide variety of patients, families and staff. (Preferred) Able to communicate effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting. Must be well organized, detail oriented, flexible and able to function independently. (Preferred) Proficient with IV’s. (Preferred) Working Conditions Lifting Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts). Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally. Ability to move around area with frequent sitting. Bending/ stooping/ squatting/ reaching/ kneeling frequently. Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Environmental Conditions Exposure to biohazard, body fluids and airborne particles. Must be able to complete tasks in a noisy environment. Mental/Visual Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted). Ability to communicate and exchange accurate information. Communicates effectively with the supervising RN, scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self-scheduled but communication of the clinician’s schedule to the nursing supervisor is essential. Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patient’s needs. Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend. Notifies the supervising RN with requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring. Suggests Community Services that may be available to the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process as requested. Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function. Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees. Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence.
