This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. Offers are contingent upon passing a background check. Responsibilities: Execute physician prescribed plans of care. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care. Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs.
