Job Requirements POSITION SUMMARY The Home Health Senior Liaison is responsible for driving business development, managing referral relationships, ensuring regulatory compliance, and identifying appropriate patients for Health First’s Home Health services. The Home Health Senior Liaison support short- and long-term planning, manages referral and expense tracking systems, and collaborates with internal and external stakeholders to promote growth and service excellence. The Home Health Senior Liaison performs referral source and patient education regarding home health service requirements. The Home Health Senior Liaison will be responsible for exceeding targeted dollar growth expectations and business development objectives beyond that of the non-senior roles. PRIMARY ACCOUNTABILITIES Develops and maintains relationships with referral sources throughout Brevard County, Florida, negotiating relationship contracts, relationships, and partnerships. Identifies and assesses patients for home health eligibility based on Medicare and payer guidelines through continuous rounding. Tracks and analyzes referral volume, conversion rates, and outreach ROI. Manage databases for referral activity and marketing expenses while resolving referral source issues and ensuring high levels of customer satisfaction. Participates in community events, health fairs, and professional networking. Ensures adherence to compliance with requirements with federal, state, regulatory bodies and Health First policies and procedures. Contributes to strategic planning and goal setting for the home health division. Collaborates with internal departments and external partners to drive growth. Provides leadership in critical thinking, conflict management and problem solving Partners with Home Health leadership to engage in patient satisfaction and increased star ratings. Coordinates and conducts in-services and continuing education programs for referral sources and their support staff as needed. Works with Intake Department for smooth on-boarding of patients in a timely manner for appropriate services. Work Experience MINIMUM QUALIFICATIONS Education: Bachelor’s degree. Work Experience: Five (5) years’ experience in home health, healthcare marketing, or referral development. Three (3) years’ experience in sales and customer experience roles within the healthcare environment. Licensure: None Certification: None Background Check: Satisfies the State of Florida Agency for Healthcare Administration (ACHA) Level II background screening. Skills/Knowledge/Abilities: Strong understanding of Medicare home health criteria and regulatory standards. Experience with referral tracking systems and expense database management. Excellent interpersonal, communication, and problem-solving skills. PREFERRED QUALIFICATIONS Work Experience: Three (3) years’ experience in sales and customer experience roles within the home health environment. Prior experience in referral management platforms. Licensure: Clinical background (RN, PT, OT, etc.). Skills/Knowledge/Abilities: Must have excellent customer service skills (both verbal and written) and have effective interpersonal skills that would allow for positive interactions with varied personality types. Proficient computer skills Proficiency in Excel, data visualization tools, and financial tracking software, such as Trella. Must maintain a professional image and abide by company dress code Knowledge of local healthcare market dynamics. PHYSICAL REQUIREMENTS Majority of time involves sitting or standing; occasional walking, bending, and stooping. Long periods of computer time or at workstation. Light work that may include lifting or moving objects up to 20 pounds with or without assistance. May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
