Teaches hospice philosophy, general eligibility requirements, and transition process to skilled home health staff, and other internal teams as appropriate, to facilitate coordination of care for transitioning patients; provides support to educate referral sources about transition program scope and guidelines. Provides coordination and support to the hospice branch for the admission visits. What is Required?
