Summary: The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants to ensure high-quality care in compliance with federal, state, and local regulations. Key responsibilities include planning and overseeing daily nursing activities, administering medications, completing required documentation, and maintaining patient confidentiality. The LPN also performs professional nursing services such as catheterization, wound care, and IV care, while fostering a warm and collaborative environment. Additionally, the role involves coordinating with physicians, ensuring safety protocols are followed, and maintaining effective communication with residents, families, and staff to meet residents' needs. Responsibilities, including but not limited to: "Ideal employee" is someone who knows the relevant industry content and can apply that knowledge to complete the tasks of their assigned job role. Be able to perform all functions under your active license as it pertains to your assigned duties. Know the residents you serve, providing care based on their care plans and physician orders. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. Assist in all aspects of a resident’s daily life, including activities of daily living, meal service, and social engagement. Appropriately communicate with clinical leadership when a resident’s needs change. Know how to approach and care for residents with dementia. Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. Experience in Long Term Care is preferred. Willing to take additional training or education, which specific states may require. CPR and First Aid certifications are required. Ability to be flexible and adaptable while handling multiple priorities. Possess written and verbal skills for effective communication. Competent in organizational and time management skills. PHYSICAL REQUIREMENTS Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Make daily rounds to ensure nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds to all customers daily. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Qualifications & Requirements Education Graduate from an accredited school of nursing.
