Communicates monthly with administrator to receive re-assessment schedule for SOURCE and CCSP annual review. 5. Review medications for appropriateness. Makes note of any new medications, dosages, and discontinued medications. 6. Documents assessment entries into Harmony system within guidelines established by the agency and/or regulatory groups. Requests medical and hospital records as needed to assist with determining eligibility. 7. Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status. 8. Attends and participates in mandatory in-services. 9.
