In addition, the Day Center Supervisor plans, organizes, directs and participates in comprehensive care tailored to the general and individualized needs and limitations of frail elderly program participants of the Healthy Aging and Wellness Program, including PACE, Assisted Living, and Adult Day Services. This role provides leadership, supervision and support to the Recreational Therapy team. As a Registered Nurse /Licensed Practical Nurse is responsible for providing clinical expertise, supervision, and support to the staff. This includes direct clinical care, family/caregiver education, oversight of Caregivers, and working collaboratively with an Interdisciplinary Team (IDT) to provide holistic care for our participants. This position is a member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), as the Day Center Supervisor and backup for the Personal Care Attendant Representative. As an RN or LPN, Day Center Supervisor is responsible for day center coverage, services, managing risk, medication management, and overall programming. Education - Associate’s Degree or higher in Nursing.
