Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. The LPN will also provide patient care by administering medications, processing patient phone calls per practice protocols, instructing patients regarding health care and treatments, and providing health care provider with information pertaining to patients' health status. May be required to float to other UFHP practices, as needed, for staffing. Qualifications Minimum Education and Experience Requirements: Licensed LPN eligible to practice in the State of Florida. One year of ambulatory care or hospital experience preferred. Person should display initiative and have the ability to multitask. Outstanding patient/customer service skills, verbal clarity, team focus, and problem-solving skills are required for the ideal candidate. Licensure/Certification/Registration: Current Licensed LPN eligible to practice in the State of Florida. Florida is part of the Nurse Licensure Compact (NLC) Act. Weekend Night Shift Supervisor (LPN/RN)

CareCore at Mentor is seeking a dedicated Weekend Night Shift Supervisor to join our leadership team. If you're an experienced LPN or RN with a passion for long-term care and leading clinical teams, this is an excellent opportunity to make an impact while working a consistent weekend schedule. Schedule Weekend Nights Friday, Saturday & Sunday | 7:00 PM – 7:00 AM Additional shifts may be available based on facility needs. What You'll Do Provide clinical leadership and supervision during weekend night shifts. Support and mentor nursing staff while promoting a positive team environment. Ensure high-quality resident care and compliance with state and federal regulations. Respond to changes in resident conditions and assist with clinical decision-making. Communicate effectively with physicians, residents, families, and interdisciplinary team members. Complete documentation accurately and ensure PointClickCare (PCC) documentation is maintained. Participate in admissions, incident follow-up, and other clinical responsibilities as needed. Serve as the nursing leader on duty during assigned shifts. Qualifications Current Ohio LPN or RN license in good standing. Previous long-term care or skilled nursing experience required. Supervisory or charge nurse experience preferred.
